Job Summary

Our Health Care Technicians, Palliative and/or Acute Medically Complex, have unique opportunities to serve clients in palliative and/or acute medically complex settings. Under the supervision of a Registered Nurse (DRN), the Care Technician provides direct, hands-on care to clients in their homes, collaborating with nursing and Home Care programs. They communicate with the DRN via a virtual electronic monitoring system, enabling real-time updates. The position is eligible for a $3.00/hour wage enhancement from the Ontario Government for all hours spent directly caring for clients. Training will be provided to support advanced skill development.

Key Responsibilities
  1. Observe and document clinical data in the electronic medical record under the direction of the DRN, following VON guidelines.
  2. Provide personal care supporting activities of daily living and medication administration under the DRN’s supervision.
  3. Maintain direct contact with the DRN through a virtual real-time electronic monitoring system.
  4. Contribute to developing and modifying care plans based on patient outcomes reported to the DRN.
  5. Identify additional resource needs in the home (e.g., home support) and coordinate scheduling with the eShift office.
  6. Communicate and collaborate with patients' families/caregivers to ensure a positive experience under the DRN’s guidance.
External and Internal Relationships
  • Frequent communication with Registered Nurses.
  • Engagement with clients and their families.
  • Interaction with eShift administrative staff.
Education, Designations, and Experience
  • Personal Support Worker (PSW) certification.
  • Developmental Support Worker (DSW) certification considered.
  • 1-2 years’ experience as a PSW in palliative, pediatric, or acute care (hospital).
  • Current CPR and First Aid certifications.
  • Knowledge of Palliative Care Level I (training or commitment to complete within first year).
  • AHPCE course completion is an asset.
Skill Requirements
  • Ability to learn and demonstrate competency in clinical skills supporting activities of daily living (e.g., subcutaneous medications, G-Tube feeds, bladder irrigation).
  • Adaptability to various social and economic environments.
  • Knowledge of community resources.
  • Strong communication skills (written and verbal).
  • Basic observation and problem-solving skills.
  • Willingness to undergo ongoing skills training and apply learned skills.
  • Ability to work effectively in a multidisciplinary team, take direction, and work independently.
Other Requirements
  • Access to a personal vehicle, valid driver’s license, and proof of insurance.
  • Willingness to travel to neighboring communities.
  • Current criminal background check, including vulnerable sector screening.
  • Ability to wear Personal Protective Equipment (PPE).
Work Conditions and Physical Capabilities

Attention to detail; ability to lift, carry, bend, push, pull, grip, reach, kneel, crouch using proper body mechanics; walk, sit, stand, climb stairs; manual dexterity.
