Job summary

A leading healthcare organization is seeking Health Care Technicians to provide hands-on care to palliative and acute medically complex clients. Job Summary:

Our Health Care Technicians, Palliative and/or Acute Medically Complex, have unique opportunities to use their talents daily with palliative and/or acute medically complex clients. Under the direction of a Registered Nurse, the Directing (DRN) Care Technician provides direct, hands-on care to clients in their homes, collaborating closely with nursing and Home Care programs. The Care Technician communicates in real-time with the DRN via a virtual electronic monitoring system, enabling immediate information sharing.

Training will be provided to support advanced skill sets and requirements.

This position is eligible for a $3.00 per hour wage enhancement from the Ontario Government for all hours spent directly caring for clients.

Key Responsibilities:
  • Observe and document clinical data in the electronic medical record as per VON guidelines, under the DRN's supervision.
  • Provide personal care supporting activities of daily living and medication administration under the DRN's guidance.
  • Maintain direct contact with the DRN through a virtual real-time electronic monitoring system.
  • Contribute to developing and modifying care plans based on patient outcomes reported directly to the DRN.
  • Identify additional resource needs in the home, such as home support, and coordinate scheduling with the eShift office.
  • Communicate and collaborate with patients’ families or caregivers to ensure a positive patient experience under the DRN’s supervision.
External and Internal Relationships:
  • Regular communication with Registered Nurses.
  • Engagement with clients and their families.
  • Interaction with eShift administrative support staff.
Education, Designations, and Experience:
  • Personal Support Worker (PSW) certification.
  • Developmental Support Worker (DSW) certification considered.
  • 1-2 years’ experience as a PSW in Palliative, Pediatric, or acute hospital care.
  • Current CPR and First Aid certifications.
  • Knowledge of Palliative Care fundamentals (Level I); willingness to undertake further training within the first year.
  • AHPCE course completion is an asset.
Skill Requirements:
  • Ability to learn and demonstrate competency in clinical skills supporting daily living activities (e.g., medication administration, G-Tube feeds).
  • Adaptability to different social and economic environments.
  • Awareness of community resources.
  • Strong communication skills, both written and verbal.
  • Basic observation and problem-solving skills.
  • Commitment to ongoing skills training and implementation.
  • Ability to work effectively independently and within a multidisciplinary team.
Other Requirements:
  • Access to a personal vehicle, with a valid driver’s license and proof of insurance.
  • Willingness to travel to neighboring communities.
  • Current criminal background check including vulnerable sector screening.
  • Ability to wear Personal Protective Equipment (PPE).
Working Conditions and Physical Demands:

This role requires attention to detail in a dynamic environment, involving physical activities such as lifting, carrying, bending, reaching, and kneeling, with emphasis on proper body mechanics. Tasks include walking, sitting, standing, climbing stairs, and fine hand movements.
