Job Summary

The Health Care Technician (HCT) provides direct hands-on care to patients in their homes under the supervision of a Registered Nurse (DRN). The role includes supporting pediatric, palliative, complex, and chronic care patients within the eShift programs, offering enhanced personal support.

  • This position is eligible for a $3.00/hour wage enhancement from the Ontario Government for all hours spent directly caring for clients.

PSWs, DSWs, HCAs, and internationally educated nurses are encouraged to apply! Training will be provided to develop advanced skills.

Health Care Technicians specializing in Palliative and/or Acute Medically Complex care at the Brantford, Haldimand/Norfolk site may be scheduled to work anywhere within these regions. Access to a personal vehicle is required.

Key Responsibilities

  • Observe and document clinical data in the patient's electronic medical record as per VON guidelines, under the direction of the DRN.
  • Provide personal care supporting activities of daily living and medication administration.
  • Maintain direct contact with the DRN through a virtual electronic monitoring system.
  • Contribute to developing and adjusting care plans based on patient outcomes reported to the DRN.
  • Identify additional resource needs in the home and coordinate scheduling with the eShift office.
  • Communicate and collaborate with patients' families to ensure a positive experience, under the DRN's guidance.

External and Internal Relationships

  • Regular communication with Registered Nurses.
  • Engagement with clients and their families.
  • Communication with eShift administrative staff.

Education, Designations, and Experience

  • Personal Support Worker (PSW) certification.
  • Developmental Support Worker (DSW) certification considered.
  • 1-2 years of PSW experience.
  • Current CPR and First Aid certifications.
  • Knowledge of Palliative Care Level I; willingness to complete a course within the first year.
  • Experience in palliative or hospital acute care preferred.
  • AHPCE course completion is an asset.

Skill Requirements

  • Ability to learn and demonstrate competency in clinical skills supporting daily activities (e.g., medications, G-tube feeds, bladder irrigation).
  • Adaptability to various social and economic environments.
  • Knowledge of community resources.
  • Strong communication skills.
  • Basic observation and problem-solving skills.
  • Commitment to ongoing skills training and application.
  • Ability to work independently and in a multidisciplinary team.

Other Requirements

  • Access to a personal vehicle, valid driver's license, and proof of insurance.
  • Willingness to travel to neighboring communities.
  • Current criminal background check, including vulnerable sector screening.
  • Ability to wear PPE.

Physical demands include lifting, carrying, bending, reaching, kneeling, walking, sitting, standing, and stair climbing, emphasizing good body mechanics.
