Job Summary

The Health Care Technician (HCT) provides direct hands-on care to patients in their homes under the supervision of a Registered Nurse (DRN). Within the eShift programs, the Care Technician offers enhanced personal support to diverse patient populations, including pediatric, palliative, complex care, and chronic disease management.
**Training will be provided to support advanced skill sets and requirements.**

Key Responsibilities

  • Observe and document clinical data in the patient's electronic medical record as per VON guidelines, under the direction of the DRN.
  • Provide personal care supporting activities of daily living and medication administration, under the DRN's supervision.
  • Maintain direct contact with the DRN through a virtual real-time electronic monitoring system.
  • Contribute to developing and modifying care plans based on patient outcomes reported to the DRN.
  • Identify additional resource needs in the home (e.g., home support) and coordinate scheduling with the eShift office.
  • Communicate and collaborate with the patient's family/caregivers to ensure a positive experience, under the DRN's guidance.

External and Internal Relationships

  • Regular communication with Registered Nurses.
  • Engagement with clients and their families.
  • Interaction with eShift administrative support staff.

Education, Designations, and Experience

  • Personal Support Worker (PSW) certification.
  • Developmental Support Worker (DSW) certification considered.
  • One to two years' experience as a PSW.
  • Current CPR and First Aid certifications.
  • Knowledge of Palliative Care Level I (training or willingness to complete within the first year).
  • AHPCE course completion is an asset.

Skill Requirements

  • Ability to learn and demonstrate competency in additional clinical skills (e.g., subcutaneous medications, G-Tube feeds, bladder irrigation).
  • Adaptability to different social and economic environments.
  • Awareness of community resources.
  • Strong communication skills, both written and verbal.
  • Basic observation and problem-solving skills.
  • Commitment to ongoing skills training and implementation.
  • Ability to work independently and as part of a multidisciplinary team.

Other Requirements

  • Access to a personal vehicle, with valid driver's license and insurance.
  • Willingness to travel to neighboring communities.
  • Current criminal background check, including vulnerable sector screening.
  • Ability to wear Personal Protective Equipment (PPE).

Physical demands include lifting, carrying, bending, reaching, kneeling, walking, sitting, standing, and climbing stairs, emphasizing good body mechanics.
