  • We have several positions available within our eHomecare program, including full-time, part-time hours.

Job Summary:

The Personal Support Worker provides personal support and homemaking services to clients and families within the framework of VON policies, procedures, standards. The PSW collaborates with members of the health care team in meeting the needs of clients to attain an optimal level of independence and well-being.

Training will be provided to support advanced skill sets and requirements.

This position is eligible for the $3.00 per hour wage enhancement from the Ontario Government for all hours spent directly caring for clients.

Available Program:

eHomecare:

  • Providing supportive care for palliative/pediatric clients in their homes.

Key Responsibilities:

  • Observe and evaluate the client’s or family's home, personal situation, and care plan to determine if any changes are needed, and advise the supervisor accordingly.
  • Contribute to the development of the personal support care plan under the direction of the Personal Support Program Manager, based on information from the funder, manager, and client or family.
  • Implement changes to the care plan under the guidance of the Manager for safety, liability, and financial reasons.
  • Report any changes in the client’s or family's needs or home situation that necessitate a change in the care plan.
  • Perform personal support tasks such as light cleaning, meal preparation, clothing care, shopping, and household safety monitoring, reflecting the needs of the client and family.
  • Provide personal care as directed, including assistance with walking, stairs, bed transfers, eating, dressing, and hygiene.
  • Provide bedside care under the direction of a nurse when required.
  • Provide respite care to clients as needed.
  • Support clients’ families by providing child care to allow for family relief.
  • Organize daily activities and set priorities for personal support and other activities.
  • Complete daily call checks and other forms to ensure effective operation of services.
  • Assess the client’s communication and relational abilities.
  • Communicate with health team members regarding homemaking, physical, and emotional needs, and inform supervisors of any concerns.
  • Comply with Occupational Health and Safety legislation, regulations, and VON policies.
  • Follow risk management policies to promote client safety.
  • Perform other duties as assigned.

External and Internal Relationships:

  • Engage with supervisors and managers regularly regarding client plans and relevant information.

Education, Designations, and Experience:

  • Certificate in Personal Support Worker, Developmental Services Worker diploma, or Home Support Worker (Level I, II, III), or Health Care Aide.
  • Current Standard First Aid or CPR Level C certificate, if applicable.
  • Experience with household management, elderly, ill individuals, or children.
  • Completion of Fundamentals of Palliative Care – Level 1 or willingness to complete within the first year.
  • AHPCE course completion is an asset.

Skill Requirements:

  • Strong verbal and written communication skills.
  • Effective communication with clients and families, demonstrating respect, empathy, and understanding.
  • Good assessment skills and awareness of community resources.
  • Ability to work independently or in a multidisciplinary team.
  • Ability to follow directions and adapt to different social and economic environments.
  • Adherence to confidentiality and PPE requirements.

Other Requirements:

  • Access to a personal vehicle, valid driver’s license, and proof of insurance.
  • Satisfactory Criminal Records Check and possibly Vulnerable Sector Search or Child Abuse Registry Check.
  • Availability for flexible hours.

Work conditions include attention to detail, lifting techniques, physical movements, and body mechanics.
