POSITION SUMMARY:

The Personal Support Workers – Collaborative Care Model (PSW-CC) enhance the quality of life of SJHC clients by providing quality and compassionate care in the form of light to moderate personal care assistance, physical and cognitive support and home keeping assistance. PSW-CC assists in enabling individuals to continue to live independently in their own homes in a safe and healthy manner.

Collaborative Care Model clients are typically older adults living in their own homes, and who have very stable but long term chronic medical conditions, or more than one medical condition, and who may have mild cognitive impairment. These clients are capable of independent living with the addition of community links, typically have a support network of other care givers, and receive a maximum of 7 hours per week of care.

RESPONSIBILITIES:

  • Assisting clients with daily personal care activities such as bathing, dressing, transfer, shaving, personal hair care, mouth care & preventative skin care, in keeping with their individual care plan.
  • Assisting clients with daily housekeeping activities such as light housekeeping and meal preparation
  • Assisting clients in daily activities which maintain adequate levels of mental / physical health and safety, in keeping with their individual care plan
  • Engaging clients in positive/social conversations
  • Encouraging clients to direct and/or participate in their own care
  • Assisting clients with general mobility needs (walking, movement, positioning etc.) in and around their homes
  • Demonstrating sensitivity and respect to clients who may be experiencing cognitive, physical, and/or mental health challenges
  • Providing emotional and social support to both clients and their family members
  • Demonstrating a positive attitude and approach towards clients as unique individuals
  • Performing daily work in keeping with the established Collaborative Care Model procedures, processes and expectations
  • Observes and records client information/comments/issues/behaviours/changes in client charts located in the home, as well as daily reporting back to Manager – Marketed Services
  • Reports critical client care information/issues to Manager – Marketed Services right away
  • Understands and follows individual client care plans, seeks clarification from Managers where care plan in unclear
  • Participating in team/group meetings
  • Other duties as requested or required

QUALIFICATIONS:

  • PSW Certificate from an accredited training institution, required
  • 1-3 years PSW experience, preferred
  • Member of the PSW Registry, preferred (may be required under some contracts)
  • Current First Aid and/or CPR Certificate, preferred
  • Training in gentle persuasive/non-violent crisis prevention, an asset
  • Previous experience working in a Hub model of care, as asset
  • Access to reliable transportation and vehicle insurance, required
