Job summary

Halton Region is seeking a Personal Support Worker to join the Social & Community Services team at Post Inn Village Long Term Care Facility in Oakville. You will provide care in activities of daily living and report changes to registered staff.

The position requires successful completion of a PSW program listed on the HSCPOA registry or current registration with HSCPOA in good standing, along with a diploma and PSW certificate from a recognized college.

Qualifications

• Must have PSW program completion on HSCPOA registry or be registered with HSCPOA in good standing.
• Secondary school diploma and PSW certificate from a recognized college.
• Ability to work in a team and communicate effectively.
• Understanding of Residents’ Bill of Rights and LTC standards.
• Ability to work independently and adapt to resident needs.

Responsibilities

• Provide personal care for residents and assist with daily living activities.
• Report changes in resident status to registered staff.
• Communicate effectively with residents, families and team.
• Maintain a home-like atmosphere and respect residents.
• Observe and document data and care needs as required.
• Share responsibility for equipment and residents’ belongings.

Job Summary

Participate as a member of the nursing and personal care team, providing care in activities of daily living to the residents. This includes reporting changes in resident status in Point of Care and carrying out direction from registered staff.

Duties & Responsibilities

• Providepersonal care for residents including all activities of daily living to ensure their optimum physical, social and emotional well-being.
• Communicateeffectively with residents, staff and families.
• Provideexcellent customer service at all times. Is respectful, courteous and pleasant manner.
• Provideindividualized service and creates a home-like atmosphere.
• Observeand reportchanges in the resident’s status.
• Recordnecessary data (e.g. flow sheets).
• Shareresponsibility for the care of equipment in the Home and the personal belongings of residents.
• Workco-operatively with team members.
• Receivedirection from registered staff.
• Participatein quality improvement activities as directed.

Skills & Qualifications

Essential

In accordance with provincial requirements established by the Health and Supportive Care Providers Oversight Authority (HSCPOA), applicants must have successfully completed a Personal Support Worker (PSW) program listed on the HSCPOA’s approved programs registry with a completion date of July 1, 2014 or later, or be currently registered with the HSCPOA in good standing.

• Secondary School Graduation Diploma and a Personal Support Worker Certificate from a recognized college.
• Team oriented with excellent communication skills.
• Understanding of the Residents’ Bill of Rights and Compliance Standards as set out by the Ministry of Long-Term Care.
• Ability to work independently, prioritize and adapt to resident needs

Preferred

• Previous experience in geriatrics, rehabilitation or care setting.
• Training in Gentle Persuasive Approach (GPA)
• Familiarity with basic computer functionality, experience with Point Click Care (PCC), or Point of Care (POC)
