Job summary

Alberta Health Services is seeking a Licensed Practical Nurse (LPN) Personal Support Coordinator for Elk Point Healthcare Centre in Elk Point, AB. You will be part of an interprofessional team providing personal support services within Long Term Care.

You will plan, organize and deliver safe, patient- and family-centered care, supervise staff, and communicate with families and health professionals in a supportive role. This regular part-time position offers meaningful work in a community setting.

Qualifications

• Completion of an accredited LPN education program.
• Active or eligible for registration with the CLPNA.
• Current Basic Cardiac Life Support - Health Care Provider (BCLS-HCP).
• Minimum 2 years' recent LPN experience in seniors health or long-term care, plus supervisory/leadership experience.

The Elk Point Healthcare Centre has an opportunity for a passionate Personal Support Coordinator (PSC) dedicated to patient- and family-centered care. This role will be an integral part of the multi-disciplinary team in Long Term Care which is made of 30 beds. You will build meaningful relationships as you work with clients daily providing personal assistance and support services to those in your care. Superior interpersonal skills will be used daily as you work closely with clients, families, caregivers, and health professionals. You will help maintain a safe environment, observe and report on patients' health and well-being, and ensure effective communication. Your role includes direct client care demonstrating genuine interest and respect for clients and families in your care. Required Qualifications:

Completion of an accredited Licensed Practical Nurse education program.Active or eligible for registration and practice permit with the College of Licensed Practical Nurses of Alberta (CLPNA).Current Basic Cardiac Life Support - Health Care Provider (BCLS-HCP).

Additional Required Qualifications:

Minimum 2 years' recent experience as an LPN in Seniors health or long-term care. Previous experience supervising or leading staff. Experience providing direction, teaching, mentoring, or coaching Health Care Aides. Demonstrated experience working within an interdisciplinary team environment.

Preferred Qualifications:

Non-Violent Crisis Intervention (NVCI). InterRAI Long Term Care. Facilities NZ Enhanced Care Planning training, Supportive Dementia Approaches and Palliative PEARLS.
