A community support organization is seeking a part-time Personal Plan Facilitator in Sudbury, Ontario. Overview

Personal Plan Facilitator - Part-Time (20 hrs/week)

Rate of Pay: $29.48 - $33.30/hr

Application Deadline: September 28, 2025

Location: Sudbury/Algoma

The Personal Plan Facilitator works with the person using services, their circle of support (including the Karis Disability Services’ team(s) that provides support) to learn about the person and use this information to facilitate the creation of a comprehensive Personal Plan. The plan will address the person’s needs, hopes and dreams with an eye toward building communities of belonging in accordance with the core Values of Karis Disability Services; Valuing People, Fostering Belonging, Serving Others and Respecting Gifts.
