The Patient Care Coordinator facilitates admission, discharge, and patient transfer processes, and is responsible for coordinating patient/resident care when necessary. The role involves managing patient flow through efficient bed utilization, ensuring adequate staffing, and supporting high-quality care. The Patient Care Coordinator also assists Program Leaders in maintaining care standards and manages operations in the absence of managers and senior staff.
