Detailed Overview

In accordance with the British Columbia College of Nurses and Midwives (BCCNM) standards of practice and the Mission and Values of Fraser Health, the Patient Care Coordinator provides clinical supervision, guidance and support to nursing staff, other clinical team members and support staff within the assigned unit(s). The Patient Care Coordinator has responsibility for the day-to-day operations of the assigned unit(s) including daily patient flow. Supports/participates in research, policy, quality improvement and risk management activities.


Responsibilities


  1. Applies established criteria and prioritizes admissions/intake, leads and facilitates actions related to daily patient flow, plans for patient discharge and appropriate patient placement/referral.
  2. Supervises nursing and nursing support staff and coordinates the work of the clinical team overall on a day-to-day basis; observes work and corrects where necessary, monitors the quality of care and the safety of patients.
  3. Supports quality patient care and excellence in practice by promoting interprofessional collaboration and team building.
  4. Initiates/leads/supports plan of care conferences and rounds for optimal patient care management.
  5. Identifies learning requirements of staff and provides input into the development of learning objectives.
  6. Acts as a point person for patients and families who have concerns or questions regarding their care.
  7. Participates in quality improvement and risk management activities.
  8. Assists the Manager/Director by attending panel interviews and providing recommendations on the selection of new staff.
  9. Promotes staff awareness and involvement in research/evaluation activities.
  10. Collaborates with internal and external stakeholders.
  11. Participates in the review, development, implementation and evaluation of clinical decision support tools/policies.
  12. Performs other related duties as assigned.

Qualifications

Education and Experience

Current practicing registration as a Registered Nurse with the British Columbia College of Nurses and Midwives (BCCNM). Three (3) years of recent related clinical experience in the designated clinical area, including one (1) year of supervisory experience, or an equivalent combination of education, training and experience.
