A healthcare services company is seeking a Patient Care Coordinator in Oakville, Ontario. We are seeking a Full-Time Patient Care Coordinator to join our team in Oakville, Ontario, starting December 2025 or January 2026.
In this role, you will serve as the primary point of contact for residents and families in Ontario and Alberta, providing day-to-day support and ensuring seamless communication between residents, their care providers, and our operations team. You’ll be the person residents and families turn to when they have questions, need help coordinating services, or require assistance navigating their care plans.
This is a highly resident-facing role where you will balance communication, coordination, and administrative support to ensure that residents receive timely, compassionate, and well-organized care.
