Patient Care Coordinator – Full-Time Status: Full-Time

Perennial delivers integrated primary care and wellness services directly within retirement residences, combining on-site and virtual care to meet seniors where they are. By proactively anticipating the needs of older adults, we improve quality of life, reduce unnecessary hospital visits, and ease the strain on the public healthcare system.

Our interdisciplinary team including physicians, nurse practitioners, registered nurses, physiotherapists, occupational therapists, dietitians, and massage therapists works together with one clear mission: to keep residents healthy, active, and comfortable in their own homes.

About the Role

We are seeking a Full-Time Patient Care Coordinator to join our team in Oakville, Ontario, starting December 2025 or January 2026.

In this role, you will serve as the primary point of contact for residents and families in Ontario and Alberta, providing day-to-day support and ensuring seamless communication between residents, their care providers, and our operations team. You’ll be the person residents and families turn to when they have questions, need help coordinating services, or require assistance navigating their care plans.

This is a highly resident-facing role where you will balance communication, coordination, and administrative support to ensure that residents receive timely, compassionate, and well-organized care.

Responsibilities
  • Resident & Family Support: Act as the “go-to” contact for residents and families in Ontario and Alberta, responding promptly to calls, emails, and texts.
  • Care Coordination: Schedule and confirm appointments with nurse practitioners, physicians, and allied health providers. Coordinate referrals and follow-ups with external specialists as required.
  • Clinical Communication: Liaise directly with primary care providers, nurse practitioners, and operations staff to ensure clear, accurate, and timely communication about resident needs.
  • Issue Resolution: Troubleshoot resident questions or concerns, escalating to clinical or operational leadership as needed.
  • Program & Service Navigation: Guide residents and families through care plans, insurance submissions, and wellness program offerings.
  • Operational Support: Partner with operations to ensure smooth day-to-day workflows across homes, including coordinating provider schedules and supporting logistics.
  • Documentation: Maintain accurate and up-to-date records in electronic medical records (EMRs) and internal systems.
  • Relationship Building: Foster trusted relationships with residents, families, and on-site staff to support a consistent, positive care experience.
  • Event & Engagement Support: Assist with resident wellness programming and on-site events in Ontario, ensuring Perennial remains a trusted partner in each community.
Qualifications
  • Experience: 2+ years in a patient-facing or healthcare coordination role (clinic, hospital, retirement, or wellness setting strongly preferred).
  • Education: Diploma or degree in healthcare administration, medical office administration, social services, or related field.
  • Skills: Excellent organizational skills, ability to multitask, and strong written and verbal communication skills.
  • Interpersonal Strengths: Empathetic, approachable, and resident-focused, with the ability to build trust and foster long-term relationships.
  • Technical Skills: Comfortable with EMRs, scheduling tools, and Microsoft Office/Google Suite.
  • Flexibility: Ability to support residents and families across multiple provinces, with occasional travel to homes in Ontario if required.
  • Languages: Additional languages are an asset given the diverse senior population.
