About The Position

The Patient Care Coordinator is the first point of contact, providing general information to patients and the public regarding their questions, concerns, and suggestions. This role facilitates the opioid agonist therapy treatment program and collaborates with the interdisciplinary team to plan, implement, coordinate, and evaluate treatment plans supporting patient recovery.

Your Duties And Responsibilities
  • Collaborate with the interdisciplinary team to plan, implement, coordinate, and evaluate treatment plans.
  • Supervise urine drug screening and testing using dipsticks.
  • If trained, perform venipuncture, ECGs, and assess vitals as required.
  • Manage patient active prescriptions, including cancellations per protocol.
  • Maintain competency in Electronic Medical Records, including verification of health card, scheduling, data, and document management.
  • Communicate with MRP, Pharmacy, and Nurse regarding patient needs and emergencies.
  • Facilitate clients in OTN sessions and conduct CARE calls.
  • Respond to inquiries and make referrals within the healthcare team.
What You’ll Need To Have
  • Strong commitment to working with individuals living with addiction and mental health issues.
  • High School Diploma and/or GED or equivalent.
  • Certificate in Medical Office Administration or Health Services is an asset.
  • Experience in Customer Service or with vulnerable populations is an asset.
  • Recent acceptable Criminal Reference Check.
  • Current First Aid and CPR Certification.
  • Proficiency with Microsoft Office applications.
