About The Position

The Patient Care Coordinator is the first point of contact and provides general information to patients and the public relating to their questions, concerns, and/or suggestions. The role involves facilitating the opioid agonist therapy treatment program and collaborating with the interdisciplinary team to support patient recovery.

Your Duties And Responsibilities
  • Collaborate with team members to plan and evaluate treatment plans.
  • Supervise urine drug screening and testing using dipsticks.
  • Perform venipuncture, ECGs, and assess vitals if trained.
  • Manage patient active prescriptions, including cancellations as per protocol.
  • Maintain competency in Electronic Medical Records, including verification, scheduling, and data management.
  • Communicate with MRP, Pharmacy, and Nursing staff regarding patient needs and emergencies.
  • Facilitate OTN sessions and conduct CARE calls.
  • Respond to inquiries and make referrals within the healthcare team.
What You’ll Need To Have
  • Strong commitment to working with individuals living with addiction and mental health issues.
  • High school diploma or equivalent required.
  • Certificate in Medical Office Administration or Health Services is an asset.
  • Experience in customer service or with vulnerable populations is an asset.
  • Recent Criminal Reference Check required.
  • Current First Aid and CPR certification required.
  • Proficiency in Microsoft Office applications.
