About the Position

The Patient Care Coordinator is the first point of contact, providing general information and facilitating the opioid agonist therapy treatment program. Collaborates with the interdisciplinary team to plan, implement, coordinate, and evaluate treatment plans supporting patient recovery.

Your Duties and Responsibilities
  • Collaborate with the team to plan and evaluate treatment plans.
  • Supervise urine drug screening and testing.
  • Perform venipuncture, ECGs, and assess vitals if trained.
  • Manage patient scripts, including cancellations.
  • Maintain competency in Electronic Medical Records system.
  • Communicate with MRP, Pharmacy, and Nurse regarding patient needs and emergencies.
  • Facilitate OTN sessions and conduct CARE calls.
  • Respond to inquiries and make referrals within the healthcare team.
What You’ll Need
  • Commitment to working with individuals with addiction and mental health issues.
  • High School Diploma or GED; Medical Office Administration or Health Services certification is an asset.
  • Experience in customer service or with vulnerable populations is an asset.
  • Recent Criminal Reference Check, First Aid, and CPR certification.
  • Proficiency in Microsoft Office applications.
