As a Patient Care Assistant, you will support the assigned team in their daily activities to ensure patients receive prompt, effective customer service. You will leverage your healthcare administrative support experience to play a key role in providing connected, accessible, patient-centered care, supported by our collaborative team of over 8,000 health professionals. Your mission will be to help our patients be healthier at home, while benefiting from our support for professional growth, wellness, and work-life balance.

Projected start date: June 16, 2025, with 6 weeks of mandatory full-time orientation.

We consider all branches of interest within HNHB; however, applicants should only apply to one branch location.

What will you do?
  • Provide administrative support services to Care Coordinators
  • Process new referrals, and orders for services, supplies, and equipment
  • Manage confidential patient records
  • Enter, update, and maintain a high volume of patient data in electronic databases
  • Answer high volumes of inquiries from patients, families, and service providers, referring callers as appropriate
  • Provide backup support to other positions as needed
What must you have?
  • A Grade 12 diploma (minimum)
  • 2+ years’ relevant office experience
  • Accurate keyboarding/data-entry skills
  • Proficiency with database software, MS Word, Excel, and other Windows applications
  • Excellent organizational skills and ability to work independently
  • Strong multi-tasking skills to meet performance and service goals
  • Exceptional interpersonal skills for working with diverse backgrounds
  • Excellent oral and written communication skills
  • Flexibility to work days, evenings, and weekends
  • Ability to pass a criminal background check

Note: All employees must submit proof of COVID-19 vaccination prior to start date.

Preferred qualifications:
  • College diploma in health, social services, business, or office administration
  • Familiarity with medical terminology and administrative procedures
  • Knowledge of Ontario Health atHome services
  • Ability to speak French or another second language
Availability Requirements

Relief employees must be available for shifts between 8:30 a.m. and 9:00 p.m., including weekends, with specific scheduling details provided. Flexibility for peak periods and holidays is required.
