Job Description:

We are seeking an experienced Nurse Practitioner (NP) for a part-time remote role focused on Transitions of Care. In this position, you will work closely with a team of experienced pharmacists to ensure continuity of care for residents as they transition across various care settings, with a primary focus on medication authorization and reconciliation.

Responsibilities:

  1. Receive medication handoffs from pharmacists and authorize medications as part of the care transition process.
  2. Conduct medication reconciliation to ensure accuracy and continuity of care across transitions.
