R OLE PURPOSE

The Licensed Practical Nurse plays an integral part in the support provided to residents within our Assisted Living program. Based at Bedford Manor, the LPN provides clinical level support and guidance to residents and staff.

STATUS AND CLASSIFICATION
Union. This position is open to applicants of all genders.

Location

SHIFT HOURS

  • Full Time (15 hours/ week)
    • Saturday and Sunday (8:00am- 4:00pm)

REPORTING AND WORKING RELATIONSHIPS

  • Reports and is accountable to the Assisted Living Manager or their designate
  • Establish and maintain effective working relationships with team members and residents
  • Function as a multi-disciplinary team member
  • Liaises with the staff and managers of Connective as necessary

KEY ACCOUNTABILITIES

  • Assesses and monitors resident /family’s actual and potential strengths and limitations including physiological, psychological, socio-cultural, and spiritual needs . Where appropriate , they encourage resident participation in activities of daily living
  • Establishes, maintains and ends a therapeutic relationship with the residents and significant others using interpersonal and interviewing techniques ensuring resident choice and autonomy in decision-making and care planning
  • Contributes to the development and ongoing changes to the individualized plan of care, including attending and providing input into interdisciplinary care planning meetings and family conferences . Advocates for the interest of the residents and their significant others . Collaborates with Community Care Clinician as required
  • Performs nursing activities and monitors/evaluates the outcomes. Performs head to toe assessments, airway management, nutrition management, infusion management, catheterizations, applying and changing simple sterile and/or non-sterile dressings, colostomy care, taking/recording vital signs, administering suppositories and enemas
  • Documents nursing decisions and outcomes and reports problems and/or changes to designated nursing staff
  • Diagnoses conditions and communicates resident status changes to the designated Community Care Clinician
  • Provides personal care to residents including assisting with bathing, dressing, care of skin and hair; changing beds; assisting with toilet needs; and overseeing exercise routines.
  • Assists with admissions, discharges, and transfers of resident s according to operating policies and procedures
  • Documents and updates all relevant information about care and procedures to achieve resident safety and comfort according to unit specific policy and procedure
  • Performs related clerical duties such as answering the telephone, making appointments, ordering supp lies and initiating diagnostic requisitions
  • Leads the orientation and training of clinical care for n ew staff (Health Care Aides - Residence Worker - Grid 10 , Assisted Living Leaders - Residence Worker, Senior – Grid 12 ) , students, and other members of the interdisciplinary team
  • Shares knowledge, provides direction, assignment and supervision to unregulated Health Care Aides ( Residence Worker - Grid 10 ) as appropriate
  • Participates in continuous quality improvement and program evaluation activities, education sessions, team conferences, meetings and committees
  • Maintains a safe environment for residents , significant others and staff by adhering to established safety policies, standards and procedures including emergency procedures and infection control guidelines . Reports on unsafe conditions and malfunctioning equipment
  • Performs other related duties as assigned

WORKING CONDITIONS

  • Function independently, while remaining a crucial member of the support team
  • Handle confidential/sensitive material with integrity and discretion
  • Adhere t o Connective’s policies and procedures
  • Adhere to the Community Care and Assisted Living Act
  • Adhere to the requirements of the Assisted Living Registrar
  • Be available to work flexible hours during the daytime, evenings, and weekends based on operational needs and requirements

QUALIFICATIONS

  • Graduation from a recognized program for Practical Nurses and one (1) year of recent, related community, acute care or long-term care experience; or an equivalent combination of education, training and experience
  • Current, full practicing licensure with the British Columbia College of Nurses and Midwives (BCCNM)

REQUIREMENTS

  • Criminal Records Checks
  • Proof of vaccination for communicable diseases may be required as mandated by Government, Health Authority, and/or funder requirements.

Compensation

The wage range for this position is $32.84 to $41.35 per hour as per collective agreement

Important: Candidates must be legally authorized to work in Canada, observe their weekly hours limit if under a temporary or study visa, and provide proof of eligibility if selected for the role.
