Access Alliance Multicultural Health and Community Services is seeking a Family Physician to provide comprehensive primary medical care across all ages within a collaborative inter-professional team. You will contribute to health promotion, disease prevention, and clinical decision-making, working within a model that supports access and equity.

The role emphasizes flexible hours, including some evenings and on-call, and a commitment to anti-racism and community health.

Qualifikationen

• Current license to practice medicine and registration with the CPSO.
• CMPA coverage required.
• 2 years+ general practice experience.
• Ability to work flexible hours, including some evenings and on-call.
• Commitment to anti-racism, anti-poverty and combating discrimination.
• Experience working in an interdisciplinary team environment.
• Excellent interpersonal and communication skills with a culturally diverse patient population.
• Proficiency with computers and clinical software applications.
• Second language preferred for catchment populations.

Aufgaben

• Provide ongoing comprehensive primary medical care for all ages, including examinations, diagnoses, counseling, screening, education and follow-up.
• Monitor and develop care plans with the team, including disease prevention and health promotion.
• Provide clinical advice to Nurse Practitioners, Registered Nurses and other team members.
• Contribute to medical directives, policies and guidelines as needed.
• Engage in interdisciplinary team-based care within the Access Alliance Model of Care and Wellbeing.
• Attend and, if required, lead regular case conferences for complex cases.
• Participate in program development, delivery and evaluation, and committee work.
• Undertake other duties as assigned.
