Job summary

Woolwich Community Health Centre in Ontario is seeking a part-time Family Physician (M.D.) to provide day and evening coverage at our New Hamburg site, joining the inter-disciplinary team to deliver comprehensive primary care.

You will provide acute, preventive, and chronic care, support patient advocacy, and participate in after-hours on-call on a shared schedule with 15 other health centre physicians and nurse practitioners, with pre-arranged hours at all facilities.

Qualifications

• Registered MD with current CPSO registration.
• 2–3 years in community-based primary care preferred; new grads welcome.
• Knowledge of community resources to facilitate referrals.
• Ability to manage multiple and complex demands.
• Team-oriented and collaborative.

Responsibilities

• Provide continuous, long-term primary care and patient advocacy.
• Provide acute, preventive, and chronic care.
• Flexibility to work one evening per week and after hours on-call on a shared schedule with 15 other health centre physicians and nurse practitioners.
