Job summary

Woolwich Community Health Centre (WCHC) in New Hamburg is seeking a part-time Family Physician (M.D.) to provide day and evening coverage as a member of an inter-disciplinary team delivering primary care.

The role emphasizes chronic and preventive care, care for seniors and families, and collaboration with nurses and other providers. Flexibility to work one evening weekly and participate in shared after-hours on-call with 15 other physicians and NPs.

Qualifications

• Registered Medical Doctor (M.D.) with current Registration with the College of Physicians and Surgeons of Ontario
• 2-3 years experience in a community-based primary care setting is preferred; new graduates are welcome to apply.
• Knowledge of community resources/services to facilitate referrals for primary health care clients
• Ability to manage multiple and complex demands
• Commitment to working collaboratively in a team environment
• Knowledge of/experience with health issues facing seniors, rural residents, youth, and families with young children
• Familiarity with PS Suite EMR considered an asset
• Treats conditions and chronic diseases and injury assessment
• Provide continuous, long-term primary care and patient advocacy
• Provide acute, preventive, and chronic care
• Flexibility to work one evening per week as well as provide after hours on-call on shared schedule with 15 other health centre physicians and nurse practitioners

Responsibilities

• Provide day and evening coverage at the New Hamburg site as part of an inter-disciplinary team
• Deliver comprehensive primary care including acute, preventive, and chronic care
• Advocate for patients and coordinate referrals to community resources
• Contribute to a schedule that includes one evening per week and after-hours on-call with peers

Job description

Woolwich Community Health Centre (WCHC) requires a part-time (0.50 FTE) Family Physician (M.D.) to provide day and evening coverage for our New Hamburg site. The Family Physician functions as a member of the inter-disciplinary team by providing a range of nursing services essential to client care.

Requirements:

Job Types: Part-time fixed term

Compensation & Benefits:

• Salary of $292,915.13 to $337,724.40 (pro-rated to working status)
• 4 weeks paid vacation (pro-rated to working status)
• 18 Sick Days per year (pro-rated to working status)
• 10 Professional Development days per year (pro-rated to working status)
• Enrolment in Healthcare of Ontario Pension Plan (HOOPP)
