Job summary

Kenora Chiefs Advisory is seeking a Palliative Care Clinical Coordinator to join the Primary Care Team. The role focuses on clinical coordination for patients and families facing chronic disease and end-of-life illness, ensuring culturally safe, holistic care within First Nations communities.

The RN will lead care planning, coordinate a multidisciplinary team, and facilitate transitions between hospital and community settings, reporting to the Director of Health.

Qualifications

• Must be a Registered Nurse in good standing with the College of Nurses of Ontario.
• Ability to work independently and in a team setting.
• Strong computer, organization, networking, interpersonal, presentation and communication skills.
• Knowledge of Anishinaabe culture; Ojibway language is an asset.
• Valid driver’s license and willingness to travel.
• Vulnerable Sector Criminal Records Check.

Responsibilities

Position Description:

The Palliative Care Clinical Coordinator is a member of Kenora Chiefs Advisory’s Primary Care Team. This position is intended to provide clinical coordination for patients and families undergoing Chronic disease and end of life illness.

Position Title: Palliative Care Clinical Coordinator

Department: Primary Care

Starting Salary: 84,365

Term: Full time

Location: 240 Veterans Drive, 3rd Floor, Kenora, Ontario

Reports to: Director of Health

Duties and Responsibilities:

• The Coordinator will lead early identification of community members who would benefit from a palliative approach, support comprehensive care planning, coordinate the multidisciplinary care team, and strengthen transitions between hospital and community care.


• Work alongside Elders, traditional knowledge keepers, nurses, physicians, nurse practitioners, and regional partners to ensure care is wholistic, culturally safe, and aligned with First nation Community values.


• Meet with allied Health Professionals within the All-Nations Health partners to coordinate care and case management.


• Meet with Patients and families to coordinate care for Chronic disease management and end of life care/ comfort care.


• Provide primary navigation support to patients, families, and caregivers, and who will improve access to existing clinical and community services.


• Provide education to patients and family members on available resources and care


• Establish relationships with Health Directors of First Nations communities


• Reduce fragmentation and improve continuity of care by establishing reliable coordination and navigation, mobilizing existing services more effectively, strengthening caregiver education and supports, and advancing a sustainability plan for comfort care across KCA communities


• Prepare summaries and reports after each session, from process content and evaluation as required by the Medical Director


• Participate in KCA Primary Care Health Team activities as required


• Contribute to the evaluation of service with respect to its effectiveness in meeting the community needs and achieving our mandate


• Additional duties as required


• Experience working with an Electronic Medical Record

Qualifications:

• Ability to work independently and in a team setting


• Excellent skills in the following areas: computer, organization, networking, interpersonal, presentation and communication (written and oral), Time Management


• Excellent communication skills.


• A full understanding of Anishinaabe culture, the ability to speak Ojibway is an asset


• University degree or college diploma from a recognized post-secondary institution as Registered Nurse in good standing with the College of Nurses of Ontario.
