Job summary

SE Health is seeking a dedicated registered nurse to provide home and community-based care, delivering comprehensive assessments and tailored care plans. You will lead patient care efforts as needed, collaborate with the care team and support services, and help clients transition smoothly to appropriate community resources.

Qualifications

  • Current registration with the College of Nurses of Ontario.
  • CNA specialty certification an asset.
  • CPR certificate required.
  • Valid driver’s license and vehicle (travel required).
  • Immunization records, clear criminal background check, and Vulnerable sector check required on hire.
  • Willingness to attain Saint Elizabeth certifications.
  • Availability to work overnights.
  • Physical requirements: lifting up to ~55 pounds, walking, stairs, bending, reaching.

Responsibilities

  • Complete comprehensive client assessments, develop care plans, implement and evaluate care based on best evidence and client needs.
  • Act as the point of care lead when designated for a client caseload, collaborating with care team and support functions.
  • Support clients through transitions by connecting them with community services.

Responsibilities
  • Complete comprehensive client assessments, develop care plans, implement and evaluate care based on best evidence/practices and on client’s unique needs.
  • When designated as a primary provider for a client caseload, act as the point of care lead accountable for their care while working in collaboration with other care team members and the support functions.
  • Support the client through care transitions by collaborating and connecting the client with community support services in their local community.
Requirements
  • Current registration with the College of Nurses of Ontario.
  • Canadian Nurses Association (CNA) specialty certification an asset.
  • Enthusiasm and love of community care and passion for evidence-informed practice & excellence in care.
  • Current CPR certificate required.
  • Valid driver’s license and vehicle (travel required).
  • Updated immunization records, clear criminal background check, and Vulnerable sector check required upon hire.
  • Willingness to attain required Saint Elizabeth certifications.
  • Availability to work overnights.
  • This is a physically demanding position and requires hand, arm and shoulder dexterity, sensory ability, lifting to a maximum of approximately fifty-five (55) pounds, walking, climbing stairs, standing, bending, reaching, and frequent movement.
