Job description

Community Facilitator - Centre Wellington/North Wellington County

Traverse Independence supports adults with acquired brain injuries (ABI), often who also have concurrent disorders such as mental health and/or addictions and who are some of our most vulnerable in the community. They are often homeless or precariously housed and challenged by all the social determinants of health. This position will be integrated within a Family Health Team (FHT) in rural Wellington County. It is an exciting position where the successful candidate will work a full team of regulated professionals at the FHT as part of the primary care team.

It is an advantage to have experience in the field of addictions and/or mental health and some experience in behaviour management and brain injury, but as brain injury is a specialized field, training both internally and externally will be offered to the successful candidate. You must have proven organizational skills, interpersonal skills, strong communication skills (both written and oral), and the ability to work as a member of a multi-disciplinary team with a comprehensive understanding of Microsoft Office products including Office 365.

Community Facilitator Requirements:

Education: Community College Diploma in Social Services, Health Care, or related discipline.

Knowledge/Skills: Demonstrated experience in behaviour management skills & knowledge of community resources.

  • Experience with people with disabilities resulting from an ABI, mental health, or addictions.
  • Applied application of computer hardware and software.
  • Demonstrated organizational skills, interpersonal skills, and communication skills both written and oral, ability to work as a member of a multidisciplinary team.
  • Must have valid driver's license and access to reliable vehicle with proof of $1,000,000 liability insurance and drivers abstract acceptable to Traverse Independence.
