Job description

Taproot Community Support Services (formerly WJS Canada) works to improve the lives of persons in their communities through a variety of programs and services.

Taproot Community Support Services provides culturally-based programs and social services for children youth and families in the Northwestern Ontario communities of Kenora. This trauma- informed program combines outreach and structured educational support to a specialized group of youth through a wrap around support system while in a safe inclusive space where participants can rebuild trust and gain the skills needed for personal and academic growth through a structured day programming and educational environment.

The Position

Assist youth to identify goals and develop plans to achieve them

Access services and navigate to connect to resources in the community as required

Provide outreach and support for vulnerable youth with complex specialized needs.

Assist youth in accessing housing education employment life skills training health and mental health and legal services.

Work collaboratively with community partners and agencies

Acting as an advocate for youth both within the agency and with external agencies

  • Develop goal focused transition plans with client;
  • Assess with client the risk factors and develop a wellness plan;
  • Develop supportive relations with clients family members internal and external staff;
  • Advocate for clients to ensure quality and timeliness of service and supports;

Create and maintain appropriate confidential records and client files complete reporting requirements in a timely manner.

Provide individual and group programming using cognitive behavioural techniques

Implement individual program plans and ensure coordination of services for each youth

Facilitate skill building experiential learning or activity group sessions such as interpersonal and life skills anger management nutrition parenting career development substance abuse etc. as required

Monitor individual progress towards goals identified with the youth and participate in discharge planning

Advocate for youth when appropriate

Involve caregivers in program planning and activities

Mediate between youth and caregivers and offer parenting guidelines and support as appropriate

Ensure safety and well being of youth involved in the program by adhering to agency policies and procedures and taking appropriate action when necessary

This position requires membership with UNIFOR.

One (1) permanent full-time position available with a weekly schedule of 40 hours :

Monday through Friday 1100-1900.

In Your Role You Will

Participate in regular staff meetings and functions as an active team member

Deliver Services according to agency policy and procedure

Maintain related documentation using agency approved software and provide reports as required

Make recommendations to the supervisor regarding program development policy and procedure formulation and program evaluation

Purchase and monitor food supplies according to approved budgets

Assist with facility maintenance food preparation and expenditure planning

Participate in agency committees upon request of the program manager

Perform miscellaneous job-related duties as required

Liaise and work collaboratively with program advisory committee

Liaise and work collaboratively with community service providers and other professionals to coordinate service provision facilitate referrals to the program and represent the organization and / or program in external events

Represent the agency on community committees as requested by the program manager

Coordinate community involvement in program activities

  • Prior to confirmation of employment all applicants must provide a current (dated within 6 months of hiring date) Criminal Record Check and Ministry Check. These checks can take up to 6 weeks to obtain please ensure you prepare accordingly.

As An Ideal Candidate You Possess

A degree or diploma in related humanities or social sciences

Minimum three years related experience

Valid class G drivers license and proof of insurability as a driver as well as reliable transportation

Current First Aid / CPR Certificates

Able to develop implement and manage outcome-based programming.

Experience and knowledge working within Youth Criminal Justice Act.

Skill in organizing resources and establishing priorities.

Able to plan and implement changes in response to client needs / desires and environmental influences through interdisciplinary case management.

Strong interpersonal communication skills and ability to work effectively with a wide range of constituencies in diverse communities able to promote cooperative work environments.

Knowledge and understanding of best practices relevant legislation guidelines policies and standards relating to service deliverables.

Able to collect organize prepare and edit correspondence reports and other documentation

Strong conflict resolution mediation and crisis prevention and intervention techniques

Commitment to quality assurance.
