CMHA Durham is seeking a Case Manager to provide comprehensive mental health support to clients at the Ontario Works Resource Centre. The role involves collaborating with clients and community resources to facilitate effective mental health services. Who we are:
Canadian Mental Health Association Durham is an integrated community mental health and primary care hub that has been providing mental health services to Durham Region for over 60 years.
During that time, it has evolved to provide a broad range of services to individuals and their families experiencing mental health challenges, from information and education to case management, psychosocial rehabilitation, housing and primary healthcare. These services are provided through inter-professional teams focused on providing clinically based services that, in addition to dealing with client mental health issues, address social determinants of health using the recovery model.
Overview: Provides case management support services to clients at the Ontario Works Resource Centre. Work collaboratively with Ontario Works staff including case managers, administration and counsellors to provide client care.
Responsibilities:
· Provides case management support services to scheduled clients and to clients present in the Ontario Works Resource Centre
· Participates as a member of a high performance, multi-service team acting as first point of contact to individuals, providing case management services and managing an assigned caseload of mental health clients
· Conducts comprehensive assessments including LOCUS assessments, follow up with referral sources and recommend services and connections to community resources
· Provides case management support services to clients and determines whether long-term services are required
· Provides case management services in an individualized, supportive and flexible manner
· Works with clients one-on-one looking at coping strategies and educating about community resources
· Advocates and assists clients to navigate the Mental Health System and link to services
· Attends workshops (in person & virtual) to provide an overview of CMHA Durham services
· Provides assistance to clients establishing peer and family support networks
· Consults and collaborates with community resources and service providers ensuring clients obtain the most appropriate services and receive continuity in their care
· Document’s client contacts, summaries and other reports in a timely manner, as outlined in the CMHA Durham policies and procedures
· Prepares and maintains statistical information
· Promoting and informing Ontario Works staff on mental health services available
· Performs other tasks as assigned by management
Skills:
· Post-secondary education in a related area of study (i.e., mental health, social sciences or human services) and a minimum of three years related experience or equivalent combination of education and experience
· Sound knowledge the Mental Health Act, Health Care Consent Act, social service benefits and related legislation in Ontario
· Ability to develop and maintain a positive therapeutic relationship with clients
· Ability to develop and maintain positive relationships with community partners and represent the agency in a professional manner at all times
· Excellent communication (written and verbal) skills
· Ability to acquire and utilize community resources effectively to meet client needs
· Ability to work with individuals of diverse backgrounds
· Excellent problem-solving skills to weigh all factors when assessing clients
· Ability to adapt to changing circumstances
· Ability to organize and present information
· Ability to work a variable schedule which may include evenings and weekends as required
· Valid Ontario Driver’s license, access to a dependable vehicle in safe working order and appropriate insurance coverage
· Knowledge of MS Office (Word, Excel, PowerPoint, and Outlook), as well as the internet
