Qualifications

• Sets priorities, provides clinical expertise and leadership for the plan of care.
• Ensures client/family participate in developing, implementing and reviewing the plan of care.
• Establishes a therapeutic relationship using interpersonal and interviewing techniques.
• Collaborates with the interdisciplinary team and initiates referrals when needed.

Responsibilities

• Provides clinical leadership in plan of care for Oncology patients.
• Leads care planning and ensures active client/family participation.
• Uses effective communication to support client autonomy in decision-making.
• Coordinates with health team and community services for referrals.
• Participates in rounds and care conferences for cohesive delivery.
