Responsibilities

Night Per Diem Hospice Registered Nurse

Mercy Hospice is a comprehensive program offered by Dignity Health for individuals and families facing serious and life-threatening illnesses with a prognosis of six months or less. Mercy Hospice professionals and volunteers offer physical, emotional, and spiritual comfort to patients and families. The primary focus is to enable families to care for their loved ones at home, with specialized support and care carried out under the direction of a physician, including:

• Regular nursing visits for pain and symptom management
• Emotional support, education, and community resources provided by a social worker
• Spiritual support by a hospice chaplain
• Personal services by certified home health/hospice aides
• Volunteer services for companionship, respite, and caregiver support
• Continuing support to families for 13 months through our bereavement program

The hospice team includes physicians, nurses, social workers, counselors, chaplains, home health aides, and volunteers. The team meets regularly to coordinate care, and additional services like pharmacy, physical, and occupational therapy are provided as needed. Hospice offers 24-hour support for patients and families.

Mercy Hospice is Medicare-certified, Joint Commission accredited, and a member of NHPCO, CHAPCA, CAHSAH, and NAHC. We participate in the We Honor Veterans program and contract with major private insurance providers. Mercy Hospice is accessible regardless of ability to pay, serving those facing life-limiting illnesses.

Provide compassionate, competent nursing care in patients' homes as part of an interdisciplinary team.

Qualifications

Minimum:

• 1 year hospice nursing (LVN or RN) experience in the past 5 years
• California RN license
• Valid driver's license; out-of-state license requires obtaining a CA license within 30 days
• Current automobile insurance and access to a vehicle
• BLS certification from the American Heart Association

Shift: 1700-0830, On Call

Preferred:

• BSN degree
• Public Health Nurse certification
• CHPN certification
