As an Occupational Therapist, you will assess and determine patient care needs and eligibility, provide access and referrals to community services, and engage with patients, caregivers and other health care practitioners.

What will you do?

The Occupational Therapist completes assessments, develops treatment plans, organizes, provides and directs occupational therapy treatment and educational programs for Ontario Health at Home Champlain clients, their families and caregivers in accordance with the standards of the College of Occupational Therapists of Ontario.

Primary Responsibilities:

  • Completes assessments of clients in a variety of treatment settings: private homes, schools, senior's residence, long term care facilities, group homes, and performs an analysis based on objective, subjective and functional findings.
  • Establishes realistic, measurable and time-limited treatment goals and develops treatment plans.
  • Ensures that occupational therapy treatments are initiated with the informed consent of the client or substitute decision maker in accordance with the Health Care Consent Act.
  • Provides direct treatment and/or support and teaching to informal caregivers, school personnel and other health care professionals involved.
  • Participates in case conferences when required and consults with other members of the health care team in fostering a holistic approach to care.
  • Completes reassessments to review patient's progress and treatment goals and reports to Care Coordinator and other members of the health care team in order to ensure continuity of care and determine if other resources and referrals are required.
  • Plans in collaboration with the patient, family and Care Coordinator, for the termination of service when patient's goals are achieved and/or when patient no longer requires the service.
  • Completes assessments and recommendations for adaptation aids, assistive devices and equipment necessary to achieve treatment goals respecting where appropriate the requirements of the Ontario Assistive Devices Program.
  • Completes documentation and maintains appropriate patient records in accordance with the standards of the College of Occupational Therapists of Ontario and Ontario Health at Home Champlain policies and procedures.
  • Acts as a resource for Ontario Health at Home Champlain employees, service providers, school personnel, long term care facilities and community by providing educational programs as requested.
  • Attends and participates actively in occupational therapy team meetings and general staff meetings.
  • Assumes responsibility for maintaining professional knowledge current and skills up to date.
  • Prioritizes occupational therapy services in accordance with the Ontario Health at Home Waiting List Management Policy.

What must you have?

  • Degree or diploma in Occupational Therapy and current certificate of registration with the College of Occupational Therapists of Ontario.
  • Related professional experience which would normally be attained over a period of two years in an Occupational Therapy role.
  • Related experience in a similar community-based setting is an asset.
  • Current CPR-C certificate.

  • Strong ability to manage difficult conversations/situations in a healthy and effective manner.
  • Demonstrated ability to work effectively in both a team environment and independently.
  • Effective communication skills to establish and maintain a wide range of contacts with professionals and organizations within the community.
  • Demonstrated strong customer service skills.
  • Effective listening and strong facilitation skills are necessary to work with patients and their families/caregivers/SDMs in order to ensure best possible service is provided to the patient.
  • Ability to deliver information effectively in a variety of settings including one-on-one, team meetings, and to assist in delivering presentations/information regarding the Ontario Health at Home Occupational Therapy program.
  • Effective planning and organizing skills to manage multiple patients, provide information reports, and take corrective action as required.
  • Ability to accurately complete required documentation, reports, forms, and recommendations.
  • Working knowledge of computer software (email, internet) and Microsoft Office applications (Word, Excel, Power Point, etc.).
  • A valid driver’s license and access to a vehicle is required.
  • Fluency in English and French is an asset.
  • Vulnerable Sector Check completed within the last six months.

Hours of Work

Monday to Friday – 8:30am to 4:30pm (35hrs/week)

We offer flexible work options; this position will have a hybrid model of work with a combination of in-office and telework.

This position is temporary until approximately January 2026.
