As a member of a specialized interprofessional team, the Social Worker/Occupational Therapist leads the team in the engagement and partnership with individuals and their family members or their natural supports, in the treatment/service planning process, and in individual and/or multiple family support therapy. In addition, expertise is provided with respect to entitlements, advocacy and "working the system". Depending on the type of referral, responsibilities may include developing and implementing individualized treatment and recovery plans, service coordination, medication management, activities of daily living, and social engagement. This may include work, interpersonal relationships, social interaction, and leisure activities that support optimal functioning and personal recovery.

In addition to supporting individual recovery, the Social Worker/Occupational Therapist contributes to regional capacity building through collaborative care initiatives and the development of local mental health resources.

Education & Experience

  • Master’s Degree in Social Work OR Master’s Degree in Occupational Therapy
  • Current and unrestricted registration with the College of Social Workers and Social Service Workers OR the College of Occupational Therapists of Ontario
  • Evidence of advanced education and training in Cognitive Behavioural Therapy (CBT), Interpersonal Therapy (IPT), and Dialectical Behaviour Therapy (DBT) (as relevant to the population served), certification preferred
  • Two (2) years’ of progressively responsible, current clinical experience providing evidence-based assessment and intervention services including experience in facilitating evidence-based groups (CBT, IPT, DBT) as appropriate for the population served, to individuals with serious mental illness within a Recovery framework
  • Experience consulting with mental health workers and community mental health agencies, in the design and implementation of evidence-based practices for individuals with serious mental illnesses
  • Experience and confidence working within a broad range of non-traditional, community settings and with a variety of service providers
  • Experience in implementing capacity enhancement and knowledge exchange activities
  • Experience and knowledge of concurrent disorders as relevant to the position
  • Experience working with transitional aged youth as relevant to the position

Position Duties & Responsibilities

  • Receives approved referrals submitted from internal and external sources
  • Screens each inquiry for urgency of need and suitability of services in order to ensure that individuals receive to the most appropriate service in a timely manner
  • Develops and maintains a therapeutic relationship with service recipients which include assertively reaching out to them in their natural environment
  • Provides service coordination for assigned individuals including coordinating, monitoring and revising the delivery of treatment, rehabilitation and support services per the individual's recovery plan and with a focus on individual strengths
  • Collaborates with the interprofessional team, to assist in the organization and management of the system of medication administration and integrates medication administration into individual recovery plans.
  • Provides ongoing assessment, problem-solving, side-by-side services, skill training, supervision (e.g. prompts, assignments, monitoring, encouragement), and environmental adaptations to assist individuals with activities of daily living
  • Assists individuals to plan and structure social and leisure-time activities on evenings, weekends and holidays, through the use of natural supports and community resources
  • Assists individuals to develop lasting connections to communities and natural supports by providing individual supportive therapy (e.g. problem-solving, role-playing, modeling and support), social-skill development, and assertiveness training to increase the individual's social and interpersonal activities in community settings
  • Identifies strengths and skills of individuals in achieving occupational goals, engagement and community participation
