Job summary

Champlain Rehab Solutions (CRS) is hiring an Occupational Therapist to deliver client-centered care in homes, retirement homes and community settings in Ottawa. You will assess, consult and implement interventions with diverse clients, coordinating with the client care team for plans and schedules.

You will document outcomes, maintain best-practice standards, and support continuous quality improvement while contributing to a flexible weekend on-call roster within the geographic region.

Qualifications

• Degree in Occupational Therapy from a recognized school.
• Member in good standing with the College of Occupational Therapists of Ontario.
• Valid Driver’s License with access to a vehicle.
• Ability to work independently and manage caseloads.

Responsibilities

• Assess and provide occupational therapy interventions in home settings.
• Consult, coordinate and communicate with client care teams on plans and schedules.
• Promote evidence-based practice and ensure documentation is complete and up-to-date.
• Contribute to continuous quality improvement and participate in on-call schedule.
