Job summary

Alberta Health Services in Chestermere is seeking an Occupational Therapist I to provide client-centered case management and OT interventions within Home Care Rural East. You will develop care plans, support independence in self-care, and collaborate with clients, families, and the inter-professional team.

Travel within Rockyview County is required; you will operate a laptop and phone, perform assessments, triage, and coordinate referrals while maintaining up-to-date documentation and quality

Qualifications

• Bachelor's degree in Occupational Therapy from an accredited university.
• Registered with the Alberta College of Occupational Therapists (ACOT).
• Three years of recent home care or acute care experience in the last five years.
• Proficient in computer use; role is heavily dependent on computer use.
• Ability to work to full scope of practice and use critical thinking.
• AADL Authorizer preferred.

Job description

Integrated Home Care uses a strengths–based Client Centered Case Management practice framework. Using a person-centered approach you will develop a care plan supporting the client/family to identify needs, focus on strengths, set achievable personal health and self-care goals and provide direct OT interventions. In collaboration with the client/family you will ensure services are provided and modified on an ongoing basis, based on client’s response to interventions, incorporating stewardship into service authorization.Reporting to the designated Care Manager within Home Care, the OT Case Manager-Interventionist works as a member of an inter-professional health care team; within the full scope of their professional practice and is responsible for assessing, planning, organizing, reviewing and evaluating the care needs of Short term acute, Acute, Wellness and Rehabilitation clients and their caregivers/families requiring community based services; fosters and promotes continuity of care and co-operative partnerships by liaising with primary care physicians, residential care facilities, acute care hospitals, and other programs/organizations involved in the provision of services.There is an expectation to be a mobile worker as this role requires travel within Rockyview County. You will be responsible to operate a laptop computer and mobile phone, and manual handling (lifting, lowering, carrying, pushing and pulling) is a regular part of the job. For example: lifting of approximately 30 pounds in and out of a vehicle on a regular basis and carrying this weight up and down stairs throughout the day is common within this job.

As an Occupational Therapist (OT), you will provide evidence-informed services, with a focus on occupational performance designed to meet the needs of individuals, groups, or a community. You may act as a direct service provider, consultant, educator, or advocate and function as an integral health care team member. You will engage individuals, groups, and communities to enable meaningful and chosen occupations, including self-care, productivity and leisure. You will support clients to achieve optimum functional abilities and independence related to their cognitive, affective, spiritual, and physical strengths and limitations. As an OT, you will provide comprehensive services, including triage, screening, consultation, assessment, diagnosis, collaborative goal setting, treatment, prevention, self-management, education, case management, referral, transition support, and collaboration with relevant and chosen stakeholders and supports within the client¿s physical, institutional, cultural, and social environments. You will include clients and families as integral members of the health care team, encouraging their active participation in all aspects of care. You will collaborate with clients and families in determining, implementing, and evaluating rehabilitation. You will participate in a variety of interprofessional teams to build awareness, enrich support and engage in quality improvement by partnering with healthcare providers, patients, families, and other stakeholders to advance services for the clients. You will contribute to quality improvement and their professional community and participate in peer mentoring and supervising students. You may also provide clinical supervision to Therapy Assistants and Therapy Aides.

Required Qualifications:
