Job summary

Pacific Coast Community Resources is seeking a Community Support Worker to provide care and support to individuals in staffed homes within the community. You will empower independence, advocate for rights, and collaborate with a multidisciplinary team to ensure high standards of care and safety.

The role emphasizes effective communication, daily living support, behavior management, and accurate health information documentation in a case management system.

Qualifications

  • Grade 12 and a valid Community Support Worker Diploma or equivalent.
  • Minimum 2 years in community social services/health care.
  • Current Emergency First Aid/CPR and Food Safe certificates.
  • Valid Class 5 driver’s license.
  • Criminal record clearance for children and vulnerable adults.
  • Eligibility to work in Canada.

Responsibilities

  • Respect the rights and choices of the people we support and advocate as needed.
  • Interact effectively with people we support.
  • Empower the people we support to be full citizens of their community.
  • Develop, implement, and document Personal Service Plans.
  • Maintain positive relationships with family, professionals, coworkers, and community members.
  • Assist with activities of daily living and medication administration.
  • Record daily activity and PHI in the case management database.
  • Keep the home clean and well maintained, including daily meals and light housekeeping.

COMMUNITY SUPPORT WORKER About the role As our Community Support Worker, you will work as part of a vibrant team to provide care and support to people living in staffed homes within their community.

You’ll encourage and empower the people we support to be independent and self-determining and to live the life they envision while maintaining excellent standards of care, health, and safety.

Your natural ability to care for others and work both independently and collaboratively as a member of a multidisciplinary team will allow you to influence your team’s success.

Core Responsibilities Ensure the supported persons’ rights and choices are respected and advocate as needed Interact effectively with people we support Empower the people we support to be full citizens of their community Participate in the development, implementation, and documentation of Personal Service Plans Foster positive relationships with persons’ family members, professionals, co-workers, and members of the community Assist with activities of daily living and medication administration Excellent understanding of behaviour management principles and strategies including mental health needs Record the daily activity and personal health information (PHI) of the people we support on our case management database To work in conjunction with the people we support to ensure their home is clean and well maintained at all times, including daily food preparation and light housekeeping duties

You will have Grade 12 education Community Support Worker Diploma or equivalent Minimum 2 years of experience in community social services/health care Current Emergency First Aid/CPR and Food Safe certificates Previous behaviour management course/training Ability to effectively communicate (verbal & written) with team members, professionals, families, and community members Non-verbal communication skills Unrestricted Class 5 Driver’s license.

If driving a commercial company vehicle, a Class 4 license is required Satisfactory Driver’s Abstract Criminal record clearance for children and vulnerable adults TB Test Moderate computer literacy Eligibility to work in Canada.
