As an Occupational Therapist (Permanent, Full-Time;Effective: Immediately; Location: Sudbury), you are responsible for the provision of occupational therapy services toOntario Health atHome patients in order to maximize the patient’s function,independence, and safety in their respective environment.

Whether you work in our office, in thecommunity, or a health care facility – you will play a lead role in providingconnected, accessible, patient-centered care – and be supported by ourcollaborative team that includes over 9,000 regulated health care and otherprofessionals.

What will youdo?

· Assesses thefactors influencing the patient’s ability to function in their environment,using a variety of standardized and non-standardized assessment tools.

· Determinepatient centered therapy goals and provides treatment program to increaseoccupational performance, independence, and safety.

· Share knowledgeand information with other health care professionals and agencies to workcollaboratively to deliver effective and efficient patient care.

· Buildstherapeutic relationships by ensuring patients’ values and preferences arerespected and balancing patient needs and choices with available resources.

Documents assessment findings, goals, andintervention plan in accordance with Ontario Health atHome policies andthe College of Occupational Therapists of Ontario standards for record keeping.

What must youhave?

RegisteredOccupational Therapist in good standing with the College of OccupationalTherapists of Ontario.
In-depthknowledge of occupational therapy services and other community resources.
Goodunderstanding of the roles of health care professionals.
Relevantoccupational therapy experience in a community/health care environment.
Comprehensiveand up-to-date knowledge of occupational therapy tools, processes,equipment, and assistive technology.
Knowledgeof funding agencies related to the occupational therapy recommendations.
Practicalknowledge and understanding of relevant legislation (e.g. regarding theprovision of health care services, privacy, health, and safety, etc.).
Comprehensiveknowledge of the standards of practice and professional guidelines setforth by the College of Occupational Therapists of Ontario.
Abilityto prioritize professional duties, manage multiple patients, andefficiently organize workload.
Stronginterpersonal and collaboration skills to work with diverse patientgroups, case managers, family members/caregivers/Substitute DecisionMakers, health care professionals, community organizations and serviceproviders.
Effectivelistening, observation, and facilitation skills.
Abilityto communicate information effectively through a variety of meansincluding reports, letters, meetings, and presentations.
Workingknowledge of computer software (email, internet) and Microsoft Officeapplications (Word, Excel).
Must have valid driver’s licenseand access to a vehicle, as regular travel within the Temiskaming Districtwill be required. Travel to other areas within the North East geographymay also be required from time to time, as the business needs require;
Advancedwritten and oral proficiency in English is essential.

What would give you the edge?

· Experience working with diversepatient groups, case managers, family members, caregivers, SDMs, health careprofessionals, community organizations, and/or service providers in CommunitySupport Services.

· Case management experience in ahealth care setting and/or Community Support Services.

· Ability to speak French oranother second language.
