Job description

Occupational Therapist - Home Care Assessment

(Locations: Belleville, Brockville, Trenton, Hamilton, Windsor, Chatham, Brampton, Barrie, Niagara, St. Catharines, Sudbury)

Lifemark’s Home and Community Care division, Community Advantage Rehabilitation (CAR), is seeking Occupational Therapists to conduct Independent Living Services (ILS) assessments across Ontario. These assessments are performed in clients' homes and are part-time, based on referrals.

The assessments include:

  1. Personal Care Assessments
  2. Home Safety Assessments
  3. Home Accessibility Assessments
  4. Mobility A and B assessments

Applicants must be in good standing with the College of Occupational Therapists and possess at least 3 years of experience. For mobility assessments, being an Assistive Devices Authorizer (ADP) is required.
