Position Summary:

As a member of the Wound Ostomy Continence (WOC) Team, the NSWOC provides specialized holistic assessment and management as part of an inter-professional team to meet the needs of individuals and families with ostomies, acute and chronic wounds, and urinary and fecal continence concerns.

Position Responsibilities:

Clinical Support and Consultation
  • Perform detailed holistic assessments for clients with wound, ostomy, and continence concerns.
  • Develop treatment plans in collaboration with the interprofessional team based on assessments.
  • Evaluate and adjust care plans, involving Wound Ostomy Resource Nurses (WORN) and K2P Wound Care Resource Nurses (K2P WCRN) as appropriate.
  • Provide education and mentorship to enhance wound, ostomy, and continence knowledge within SE Health.
  • Offer consultation to care providers within the organization.
  • Facilitate dissemination of wound, ostomy, and continence knowledge.
  • Coordinate with the interprofessional team across the continuum of care to optimize outcomes.
Program Development and Leadership
  • Support quality healthcare through teaching, mentoring, and program development.
  • Review Quality Improvement (QI) reports to prioritize needs and improve client outcomes.
  • Identify nursing trends and issues in wound, ostomy, and continence care.
  • Assist in developing educational tools and initiatives for care providers.
  • Stay updated on current evidence-based practices and promote research-based practice.
  • Identify and support research initiatives, including case studies and conference presentations.
  • Collaborate with professional nursing associations and local health initiatives.

Qualifications/Requirements:

  • Completion of Nurses Specialized in Wound, Ostomy & Continence Canada (NSWOCC) WOC-EP program (international certification considered with approval).
  • Active membership in NSWOC Canada preferred.
  • Current RN license with the College of Nurses of Ontario.
  • Flexibility and commitment to advancing wound, ostomy, and continence care.
  • CNA certification WOCC(C) preferred.
  • Willingness to participate in ongoing education.
  • Adopt SE model for wound care and mentor WORN/K2P WCRN.
  • Valid driver’s license and willingness to travel.
  • Excellent communication and teaching skills.
  • Knowledge of community healthcare is preferred.
  • Experience in teaching and developing educational tools is preferred.
  • Proficiency with computer applications and internet research.
