POSITION SUMMARY:

As a member of the Wound Ostomy Continence (WOC) Team in Downtown Toronto, you will provide specialized holistic assessment and management as part of an interprofessional team to meet the needs of individuals and families with ostomies, acute and chronic wounds, and urinary and fecal continence concerns.

Responsibilities:
Clinical Support and Consultation
  • Perform holistic assessments for clients with wound, ostomy, and/or continence concerns.
  • Develop treatment plans in collaboration with the interprofessional team.
  • Evaluate care plans, involving WORN and K2P WCRNs as appropriate.
  • Provide education and mentorship to build knowledge within SE Health.
  • Offer consultation to care providers and facilitate knowledge dissemination.
  • Coordinate with the care team to optimize services and reduce client anxiety and costs.
Program Development/Leadership
  • Support quality healthcare through teaching, mentoring, and program development.
  • Review Quality Improvement reports to enhance client outcomes.
  • Identify trends and issues in wound, ostomy, and continence care.
  • Develop educational tools and stay updated on best practices.
  • Promote research-based practice and participate in research initiatives and conferences.
  • Assist in developing conference posters and case studies.
  • Collaborate with professional nursing associations and local initiatives.
Minimum Requirements:
  • Completion of Nurses Specialized in Wound, Ostomy & Continence Canada (NSWOCC) WOC-EP program (international certification considered with approval).
  • Current membership in NSWOC Canada preferred.
  • RN with current certificate of competence with the College of Nurses of Ontario.
  • Flexibility and commitment to wound, ostomy, and continence care.
  • CNA certification WOCC(C) preferred.
  • Willingness to pursue ongoing education.
  • Mentorship ability for WORN/K2P WCRNs.
  • Valid driver’s license and willingness to travel.
  • Excellent communication and teaching skills.
  • Knowledge of community healthcare preferred.
  • Proficiency in computer skills is preferred.
