A leading healthcare institution seeks a Nurse Practitioner to deliver patient-centered care in compliance with Ontario's nursing standards. The role involves autonomous clinical practice, collaboration with healthcare teams, and requires significant nursing expertise. Candidates should possess a current certificate from the College of Nurses and related nursing experience to provide high-quality patient care.
KEY FUNCTION:
Deliver nursing care in compliance with the standards set by the College of Nurses of Ontario, as
well as organizational and unit-specific guidelines. Collaborate effectively with other healthcare
providers to ensure comprehensive patient care. Make clinical decisions and exercise judgment to
provide the highest patient within available resources.
The Nurse Practitioner will enhance the quality of patient care by fulfilling roles as an advanced
practitioner, educator, researcher, and consultant. Provide expert clinical care to manage
patients’ health needs. Operate autonomously with full accountability and the authority to
independently diagnose, order diagnostic tests, and prescribe medications and treatments in
accordance with the standards set by the College of Nurses. The role reflects a balance
between interdisciplinary collaboration and independence.
REPORTING:
Reporting to the Manager, Patient Care Clinical Services.
DUTIES:
1. Utilize advanced clinical knowledge and skills to assess and diagnose acute illnesses, injuries,
chronic diseases, co-morbidities, and emergency health needs. Order diagnostic tests,
prescribe medications and treatments, and evaluate patient care outcomes.
2. Create comprehensive treatment plans for patients and work collaboratively with the patient,
family and healthcare team to ensure coordinated, inter-professional care.
3. Refer patients to or consult with other healthcare providers when conditions fall outside the
Nurse Practitioner’s scope of practice or expertise.
4. Educate, support, and coach patients and their families about diagnoses, prognoses, treatment
plans (including risks and benefits), and self-management expectations.
5. Lead family meetings to discuss care plans, discharge processes, and end-of-life issues with
patients and their families or substitute decision-makers.
6. Develop and implement discharge plans, including dictating summaries that detail diagnoses,
recommended treatments, and discharge instructions.
7. Comply with relevant legislation, professional standards, and organizational policies related to
patient care, privacy, documentation, and information management.
8. Commit to continuous self-improvement and professional development by staying up to date
with best practices through research and literature.
9. Consistently demonstrate clinical competence in all aspects of practice.
10. Share responsibility with administrators and physicians for the effective design, operation, and
evaluation of the service provided by the program.
11. Engage in ongoing quality improvement and risk management initiatives.
JOB DESCRIPTION
12. Lead the development and implementation of medical directives, order sets, teaching
protocols, outcome standards, and patient education materials to ensure high-quality care.
13. Serve as a preceptor, mentor, and coach to nursing colleagues, other healthcare team
members, and students.
14. Exemplify professional behavior, demonstrating strong interpersonal skills and a commitment
to patient-centered care.
15. Other duties as assigned.
QUALIFICATIONS
EDUCATION AND TRAINING:
1. Current certificate of competency from the College of Nurses of Ontario as a Registered Nurse
in the Extended Class with a Primary Health Care speciality.
2. Membership with a professional Association such as Canadian Nurses Association, Registered
Nurses Association of Ontario or Nurse Practitioner Association of Ontario.
3. Participation in annual individual competency improvement in accordance with the College of
Nurses of Ontario Quality Assurance requirements.
4. Current ACLS certification and knowledge of Canadian Triage and Acuity Scale.
EXPERIENCE:
1. Three (3) years varied nursing experience including emergency and acute care.
