The Schizophrenia Outpatient Clinic (SOC) and Transitional Outpatient Program for the Schizophrenia Service (TOPSS) utilize a clinical case management model of service delivery that supports individuals with schizophrenia and psychosis to regain and manage their health, aiming to help them live meaningful lives through compassionate, innovative, and specialized mental health and addiction services.
The Nurse Practitioner (NP-PHC) is an autonomous healthcare provider who collaborates with physicians, interdisciplinary team members, clients, and families to deliver evidence-informed, integrated care to outpatients of TOPSS and SOC. The NP’s practice is grounded in professional, ethical, and legal standards, utilizing advanced nursing knowledge, skills, clinical reasoning, problem-solving, and critical thinking. The NP builds upon established RN competencies with an expanded scope of practice based on legislative authority to perform controlled acts autonomously. Core competencies include clinical practice, leadership, research, consultation, and collaboration. All duties align with the Standards of Behaviour and policies set by St. Joseph’s Healthcare Hamilton. The NP reports jointly to the Manager of SOC/TOPSS and the Director of Nursing.
