The Opportunity

The Nurse Practitioner will work collaboratively on the delivery of resident care to meet individual needs within his / her level of competency. This role provides quality and safe resident care which helps to support and align resident needs.

What Is In It For You
  • Collaborate and liaise with the MD, DOC and interdisciplinary team in care delivery.
  • Assess, diagnose, and evaluate acute and chronic illness and other changes in resident's conditions, prescribe treatments, diagnostics and follow up care.
  • Support and advocate for residents' best interests while engaging with the resident / family / substitute decision maker (SDM) regarding Care.
  • Participate in resident care conferences.
  • Provide clinical leadership to the LTC team members centered around the resident care needs which can include resident move in / out, hospital discharge planning, palliative and end-of-life care, pain management, dementia care and responsive behavior management.
  • Participate in identifying, analyzing, and interpreting trends in resident care and professional nursing practice to determine priorities for educational programming for team members, residents, and families.
What You Will Need
  • Minimum one year clinical and / or working experience in geriatric, rehabilitation, or long-term Care nursing.
  • Some leadership knowledge / experience is considered an asset.
  • Certifications obtained or in progress that include gerontology, mental health, behavioral support, and palliative care are an asset.
  • Current registration with the College of Nurses of Ontario as a Registered Nurse in the Extended Class (Primary Health Care or Adult) and entitled to practice.
  • Working Knowledge of the; Long Term Care Act, Program Standards for Long Term Care, Health Disciplines Act, Occupational Health & Safety Act, Pharmacy Act, Food and Drugs Act (Canada), Narcotic Control Act (Canada) and Coroners Act, is considered an asset.
