Nurse Practitioner, Raven Song Clinic Primary Care and Enhanced Outreach

Join Vancouver Coastal Health as a Nurse Practitioner at Raven Song Clinic, focusing on primary care and outreach for vulnerable populations. The role offers a salary range of CAD $74.48 to CAD $81.93 per hour.

This position involves providing team-based, full-scope primary care, including outreach and in-clinic services, to individuals facing complex medical, social, substance use, and mental health challenges. Knowledge in opioid agonist therapies, mental health, gender-affirming care, and complex illness management is advantageous. Vancouver Coastal Health supports new practitioners with orientation, training, mentorship, and ongoing development.

Key Responsibilities:
  • Conduct comprehensive assessments, diagnose diseases, and develop treatment plans.
  • Initiate treatments, manage healthcare, and prescribe medications within regulatory standards.
  • Provide guidance to other health professionals and work autonomously within interdisciplinary teams.
  • Collaborate with clients and teams to identify health trends and develop strategies for improvement.
  • Engage in policy development, quality improvement, and peer review activities.
  • Maintain professional development and clinical privileges.
  • Report to the Operational Director, Professional Practice, ensuring quality and safety standards.
Qualifications:
  • Current registration as a Nurse Practitioner with the BC College of Nurses and Midwives (BCCNM).
  • Experience in the field is preferred but motivated new graduates are encouraged to apply.
Skills and Knowledge:
  • Ability to perform full scope of NP duties.
  • Knowledge of developmental stages, pathophysiology, pharmacology, and evidence-based practice.
  • Excellent communication and collaboration skills.
  • Leadership and critical assessment abilities.
