POSITION: Nurse Practitioner-Primary Care & Outreach

STATUS: Full Time Permanent & Full Time Temporary Contract (12 months with possibility to extend)

About Us:

Grand River Community Health Centre (GRCHC) is a dynamic, client-centred organization committed to delivering high-quality primary health care services to the diverse community of Brantford. Our interdisciplinary team provides holistic and inclusive care, addressing the social determinants of health while focusing on prevention, wellness, and chronic disease management.

The Nurse Practitioner (NP) works with 2 focused areas of service including providing primary health care for a roster of clients at the Centre and providing primary health care for outreach clients.

The Nurse Practitioner will function with an integrated team of professionals that offer primary care including: assessment and development of treatment plans in collaboration with the client, caregiver, and referrals as required. This integrated role also provides flexible outreach services to vulnerable individuals of all ages who may be at risk of homelessness, and who may have addiction, and /or mental health challenges.

This position will require you to work a minimum of 1 evening per week and possibly weekends.

Position Summary:

We are seeking a dedicated and compassionate Nurse Practitioner (NP) to join our team. The NP will provide primary health care services to individuals and families, emphasizing providing primary care to people that are experiencing homelessness, mental health and addiction issues as well as health promotion, disease prevention, and chronic disease management.

The NP will work collaboratively within an interdisciplinary team, ensuring high-quality, patient-centered care that aligns with GRCHC’s mission and values.

Key Responsibilities:

  • Conduct comprehensive health assessments, diagnosis, treatment, and management of acute and chronic conditions.
  • Develop and implement individualized care plans in collaboration with patients and interdisciplinary team members.
  • Prescribe medications and other therapeutic interventions in accordance with scope of practice.
  • Provide health education, counseling, and support to patients and families to promote overall well-being.
  • Collaborate with physicians, social workers, dietitians, and other health professionals to ensure coordinated care.
  • Participate in quality improvement initiatives and health promotion programs within the community.
  • Maintain accurate electronic medical records and documentation in accordance with regulatory and organizational guidelines.

Qualifications & Requirements:

  • Registered Nurse (RN) with extended class certification (NP) in good standing with the College of Nurses of Ontario.
  • Minimum of 2 years of experience in primary care, community health, or a related field preferred.
  • Strong clinical skills with a focus on patient-centred, evidence-based care.
  • Excellent communication, collaboration, and problem-solving abilities.
  • Ability to work independently and within an interdisciplinary team environment.
  • Experience working with diverse and vulnerable populations is an asset.
  • Proficiency in electronic medical records (EMRs) and documentation.
  • Demonstrated experience in Community and Mental Health and Addictions
  • Ability to work autonomously as part of a multi-disciplinary team
  • Knowledge and proficiency in current, evidenced-based methods and practices of primary care delivery, with an emphasis on health promotion and harm reduction
  • Superior leadership, organization, research, evaluation, time management, communication and interpersonal skills
  • Experience working with electronic medical records (EMR)
  • This ONA position offers a competitive wage range, of $55.31 to $66.67 per hour and benefits (including HOOPP) are offered.
  • Employment is conditional upon providing a police vulnerable sector check and having had a full series of COVID-19 vaccinations
