Job summary

Flemingdon Health Centre (FHC) is seeking a Nurse Practitioner to join the Canadian Family Clinic team in North York, Ontario. This 0.5–1.0 FTE role includes evenings and weekends, with a pro-rated salary between CAD 115,018 and CAD 138,264 per year.

Qualifications

• Registered Nurse in the Extended Class with the College of Nurses of Ontario.
• Baccalaureate in Nursing and completion of a recognized Primary Care Nurse Practitioner program.
• Two to three years of primary care or community nursing experience is an asset.

Responsibilities

• Provide comprehensive primary care, including assessment, diagnosis, treatment plans, follow-up and referrals.
• Order, interpret and follow up on tests; prescribe designated drugs as authorized by the CNO.
• Maintain accurate client records and coordinate care with health teams and partners.
• Support health promotion and quality improvement initiatives.
