Health for All Family Health Team in Ontario is seeking a Nurse Practitioner (RN(EC)) to provide preventive, chronic, and acute care within a collaborative primary care setting. You will work with a multidisciplinary team to deliver patient-centered services and mentorship within the teaching unit.
Responsibilities include conducting assessments, developing care plans, managing chronic conditions, and supporting community outreach to promote health equity across diverse populations.
