Nurse Practitioner - Nurse Led Outreach Team

PositionSummary: TheNurse Practitioner (RN(EC)) for the Nurse Led Outreach Team (NLOT) program usesadvanced practice clinical skills to provide direct clinical care, education,research, consultation and treatment around Geriatrics to support the acutecare needs of residents in LTC. Thisrole is central to improving access to timely, high-quality medical care forolder adults living in LTC, reducing avoidable hospital transfers, andsupporting a more coordinated and resident-centered approach.

Position Responsibilities:

  • Conduct comprehensive health assessments, diagnose,and manage acute illnesses, and provide timely follow up care within LTC homes.
  • Prescribe medications, order and interpretdiagnostic tests, and coordinate with attending physicians and LTC staff.
  • Select appropriate interventions from a range ofnon-pharmacological and pharmacological to restore or maintain residentfunction, physiological, and mental stability to achieve optimal health.
  • Communicates with residents about health assessmentfindings and/or diagnosis, including outcomes and prognosis.
  • Document assessments, intervention plans, progressnotes, and outcomes in a timely manner.
  • Support advanced care planning, goals of carediscussions, and palliative care delivery in alignment with resident wishes.
  • Serve as a liaison between LTC homes, hospitals,specialists, and community partners to ensure continuity of care.
  • Provide clinical leadership, mentorship, and supportto LTC nursing staff to enhance capacity and care quality.
  • Participates in case conferences, family meetings,and quality improvement initiatives.
  • Capture relevant and required data as defined by OntarioHealth, Health Quality Ontario, Accreditation or other quality improvementstrategies.
  • Align improvement projects with the organization andmonitor or adjust to achieve greater outcomes.
  • Evaluate services and satisfaction on an on-goingbasis and formulate improvement strategies and solutions.
  • Perform other duties as required.

PositionRequirements:

  • RegisteredNurse Practitioner in the Extended Class.
  • Master’sdegree in nursing (MScN or MN) is an asset.
  • Registeredand in good standing with respective regulating body.
  • Minimumof 3 years’ experience working with older adults.
  • Experiencein program development and education.
  • Demonstratedknowledge of evidence-based practices.
  • Effectivetime management and ability to prioritize workload.
  • Mustpossess the ability to work with minimal supervision and must followstandardized procedures.
  • Itshould be adaptable to cope with changing contacts with people, situations andconditions.
  • Musthave an employment history which demonstrates regular attendance at work.
  • Mustpossess the ability to provide effective services to clients/patients in bothEnglish and French. Individuals who donot meet the language requirements of this post are invited to submit theirrésumé for consideration.

  • Starting salary of $60.99/hour; $118,930.50/year(As per Step 1 of the ONA collective agreement.)
