Canadian Mental Health Association Durham is seeking a Nurse Practitioner and Clinic Lead to provide comprehensive care within a community health setting. This role emphasizes health promotion and patient education while overseeing clinic operations and team management. The successful candidate will ensure high-quality patient care and support professional development within a multidisciplinary team. Who we are:
Canadian Mental Health Association Durham is an integrated community mental health and primary care hub that has been providing mental health services to Durham Region for over 60 years.
During that time, it has evolved to provide a broad range of services to individuals and their families experiencing mental health challenges, from information and education to case management, psychosocial rehabilitation, housing and primary healthcare. These services are provided through inter-professional teams focused on providing clinically based services that, in addition to dealing with client mental health issues, address social determinants of health using the recovery model.
OVERVIEW:
The Nurse Practitioner (NP) and Clinic Lead provides services to clients who include the individual throughout their lifespan, the family, and the community, with strong emphasis on health promotion, patient education and disease prevention. In addition to the clinical functions, the Nurse Practitioner and Clinic Lead participates in the development, implementation, monitoring and evaluation of programs and services in the Clinic within the Canadian Mental Health Association (CMHA) Durham with a focus on quality care and risk management.
The NP and Clinic Lead position is a shared Admin/Clinical role. This includes all expectations and duties of the Job Description of the Nurse Practitioner in addition to the description that follows. Flexibility in hours between the two roles will be maintained to meet the demands of the role.
DUTIES and RESPONSIBIITIES:
Clinic Leadership
•Works collaboratively with colleagues, co-workers and community partners to ensure the clinic fulfils its mandate for quality client care delivered in a safe environment
•Communicate the vision, mission, values and strategic plan of the organisation and the clinic to employees to ensure a clear direction and understanding of tasks
•Responsible for the operational activities of the clinic, to ensure expenses, staffing, policies and procedures and procedures are compliant with organisational expectations
•Assists the Senior Director, Clinical Operations in the development of quarterly and yearly reports to the Board of Directors and Ontario Health
•Communicate decisions of the Executive Leadership Team or the Board; to clinic employees by meetings, emails or other acceptable forms of communication.
•Complies with all workplace health and safety conditions and understands the role and the responsibility of the Supervisor under the Occupational Health and Safety Ace (OHSA).
•Creates and monitors the quality and improvement plan (QIP) and annual board report in conjunction with other clinic leadership
•Develops, updates and distributes the policies, procedures, and guidelines involving the Clinic to all employees to maintain best practice and clear communication of expectations
•Plays a lead role in Accreditation activities to demonstrate sound clinical and administrative leadership for the clinic
People Management
•Design and develop clinical position job descriptions in conjunction with Employee Experience

•Supervise, mentor and orient allied health professionals to ensure compliance and adherence to clinic and organizational standards
•Recruitment, selection, supervision and evaluation of all clinic in consultation with Employee Experience and relevant policies/guidelines.
•Recruitment of allied health professionals in collaboration with the healthcare team when required
•Encourage professional and personal development of staff
•Effectively resolves staff problems as they arise in consultation with Employee Experience when necessary
Financial Management
•Review monthly and quarterly financial reports to identify variances in expenditures
•Ensure there is a high level of integrity and accuracy in the financial information that is reported both internally and externally to maintain reputation and financial accountability
•Contributes to the growth and development of the clinic through the submission of briefing notes and reports with respect to space, staffing, equipment and technology
Team Development
•Coordinates NP and multidisciplinary team meetings on a regular basis to maintain team morale and promote communication amongst the team
•Plan and participate in regular team building activities/workshops to keep team up to date on organizational activities and support professional growth
•Supports individual growth through ongoing professional development and provides clinical guidance and support to clinical staff
•Promotes participatory management style
•Fosters an atmosphere of professionalism, accountability, shared learning, collaboration and positive outcomes to model behaviours that reflect the vision, mission and values of the organization
•Fulfils all mandatory training to stay compliant with legislation and provide sound role modeling to clinic team members
Collaborative Relationships
•Collaborates with clinical staff on clinic developments to foster team collaboration
•Liaise directly or coordinate with another team member to link with community service organizations, agencies, and hospitals to support the development of a primary health care network and reciprocal referral relationships
•Collaborate with Ontario Health and geographic leads for population planned service delivery and integrated service delivery to ensure community partnerships are active
•Develop collaborative relationships with local area NPs and the regional NPAO chapter (for patient care needs, networking, resource sharing and to meet College of Nurses of Ontario Quality Assurance expectations) to keep up to date with clinical best practices and trends
•Participate in clinical projects/studies as required to support strategic goals and promote professional development.
•Attend and participate in leadership meetings and committees as identified for the Lead role.

Client Safety
Adhere to CMHA Durham client safety programs including program policies as well as infection control and prevention procedures. In partnership with clients, family members, and fellow staff, maintain open lines of communication to assist the organization in identifying and achieving quality outcomes for client safety. Report client safety incidents and suggested improvements to manager or delegate.
Risk Management
• Minimizes risks adhering to CMHA Durham policies and procedures identifying, assessing, evaluating and controlling risks ensuring the well-being of staff and clients
•Assists in developing the skills, performance and cohesion of the Emergency Response Team
Other Duties as Assigned
• Attend, participate in team meetings and all employee meetings
• Participate in organizational quality improvement activities, e.g. accreditation projects, committee work, special events
• Participate in quality improvement activities at the team/program level
• Participate in external committees as required

EFFORT
•Sits, stands and walks daily to perform the duties of the job
•Concentration, attentiveness and creativity required daily to assess clients, administer medications/ injections, attend meetings, manage system of medicine administration, prepare documents, listen to clients, use nursing equipment, etc., as well as may lift walkers and push wheelchairs as needed
•Committed to self-care to maintain therapeutic competencies
SKILLS
•Current registration with the College of Nurses of Ontario as an RN(EC) required
•Masters in Nursing or recognized equivalent required
•Valid Basic Life Support required
•Current Membership with the Nurse Practitioner Association of Ontario (NPAO)
•A certificate in Mental Health and Addictions for Nurse Practitioners or other additional formal training in mental health is considered an asset.
•5 years leadership experience in a community, hospital or primary care setting preferred
•Satisfactory vulnerable sector and criminal reference check
•Incorporates sound theoretical knowledge and best practice guidelines into client care
•Excellent communication (written and verbal) skills with the ability to formally present information to individuals or groups
•Proficient in use of internet, Microsoft software, drug database programs, virtual meeting platforms
•Current valid Ontario Driver’s License an asset
•Experience with electronic medical records system is an asset

•Sound knowledge of the Mental Health Act, Health Care Consent Act, and other related legislation in Ontario is an asset
•Sound knowledge of psychotropic medications including usage and potential side effects
•Ability to manage a system of medication checks ensuring accuracy
•Fosters positive relationships with clients, community, partners, often with diverse backgrounds, and always represent the agency in a professional manner
•Resolves complex client problems to ensure a high standard of nursing service
•Demonstrates flexibility to adapt to change and support any function required within the clinic (i.e. injections, p phlebotomy, administrative support)
•Works independently when necessary
•Applies critical and creative thinking skills to problem solving
•Ensures accurate nursing assessments and implements best practices
•Participates as a member of a high performance multi-disciplinary team
•Intermediate proficiency in written and verbal French is an asset
WORKING CONDITIONS
•Works in a medical office environment and, as required in residential homes and/or shelters
•Exposure to clients in crisis
•Exposure to abusive/irate clients, deadlines, multiple demands, distractions/Interruptions
•Exposure to hazards such as blood and bodily fluids and cleaning fluids
•Extended clinic hours as scheduled
