Job summary

Schlegel Villages is seeking a Nurse Practitioner for the Village of Humber Heights in Etobicoke. You will join a two-NP model, providing comprehensive geriatric assessments and high-quality clinical care to residents.

The role emphasizes teaching, mentorship, and leadership within an inter-disciplinary team, with focus on clinical quality improvement and resident-centered care.

Qualifications

  • Hold a current registration as a Nurse Practitioner with the College of Nurses of Ontario.
  • Experience completing comprehensive geriatric assessments.
  • Hold a current Heart Save level of CPR.
  • Maintain a current AIS Certification.
  • Experience leading, coaching and mentoring care professionals in a healthcare setting.
  • Display a caring and respectful attitude toward residents, families and team members.
  • Understand the physical, mental and emotional demands of the role.
  • Knowledge of policies and regulations in long-term care (LTC Homes Act, 2007 and Health & Safety).

Responsibilities

  • Provide high-quality clinical care focusing on resident well-being.
  • Lead quality improvement initiatives and collaborate with interdisciplinary teams.
  • Support and guide resident-centered palliative care services.
  • Contribute to clinical reviews of Village quality initiatives.

Status: Permanent Full-Time

Location: 2245 Lawrence Avenue West, Etobicoke, M9P 3W3

Position Type: Permanent Full time

Expected Start Date:September 01st, 2026

The salary for this position starts at $113,683, and starting salary increases, commensurate with years of NP experience, to a max starting salary of $131,376 for those with 8 years of NP experience.

The Village of Humber Heights will have 2 Nurse Practitioner Model and Both NP will work together best Health Care Service to our residents.

To be successful as our new NURSE PRACTITIONER, you will:

  • Hold a current registration as a Nurse Practitioner with the College of Nurses of Ontario

  • Knowledge of relevant policies, procedures, legislation and regulations applicable to the long-term care sector, including the Long-Term Care Homes Act, 2007 and Health & Safety

As a new Nurse Practitioner (NP) joining our Village, you will:

Support and embrace our resident-centered social culture and be proud to deliver the highest quality of clinical care to our residents. NPs are focused on quality resident care and act as an educator and clinician in developing a multi-disciplinary Village team. The Nurse Practitioner will provide a focus on clinical quality improvement outcomes, working with and supporting the inter-disciplinary teams in working towards targets in improvement.

The Nurse Practitioner will provide support and guidance in completing assessments, ordering, interpreting, and communication diagnostic testing and procedures, provide support for and lead resident centered palliative care services, and participate in the clinical review of Village quality initiatives.
