Nurse Practitioner - Integrated Palliative Care (1 year contract)

Job Overview

We are seeking an experienced Nurse Practitioner to provide leadership, clinical consultation, and direct care to patients requiring hospice palliative care within Ontario Health atHome. The role involves supporting primary care providers and working as part of the Palliative Care Outreach Team (PCOT).

Responsibilities
  1. Deliver comprehensive healthcare services, including pain and symptom management, in accordance with the College of Nurses of Ontario standards.
  2. Participate in continuing education and mentorship within the organization.
  3. Adapt to changing priorities to meet patient and family needs.
  4. Collaborate effectively with system partners and adhere to safety policies.
Qualifications
  • Registration in the Extended Class from the College of Nurses of Ontario (eligible to practice as RN(EC)).
  • Certification in Hospice Palliative Care or relevant specialty preferred.
  • Master’s degree preferred.
  • 3-5 years’ experience in hospice palliative care or primary/acute care with interest in palliative care.
  • Evidence of cultural safety training and ongoing professional development.
  • COVID-19 vaccination required.
Desired Skills
  • Strong consultation skills and community engagement.
  • Knowledge of Indigenous and Francophone communities for culturally safe care.
  • Fluency in English; French language skills preferred.
