The London HART Hub is a low barrier recovery-oriented space supporting individuals experiencing homelessness with mental health and addiction needs. It provides access to evidence-informed, respectful, trauma-informed, culturally appropriate, and compassionate care, connecting clients with a continuum of services including transitional supportive housing, healthcare, and mental health and addiction services.
The Nurse Practitioner (NP) in the Hubs program will deliver integrated mental health and addiction-based medicine and primary care. Responsibilities include providing direct and indirect patient care, collaborating with inter-professional teams, and supporting mental health and addiction needs, including withdrawal management.

WHAT WE OFFER:
- Starting rate: $65.69/hour (Level 1 of Nurse Practitioner Grid); with progression up to $70.87/hour based on seniority
- Work schedule: 70 hours over two weeks, Monday-Friday 8am-4pm
- Benefits: 9% in lieu of benefits
- Pension: Immediate eligibility for Healthcare of Ontario Pension Plan (HOOPP)
HOW YOU WILL MAKE A DIFFERENCE:
- Providing services addressing addictions and general medical needs
- Offering non-urgent healthcare within scope, consulting with physicians for complex cases
- Coordinating with psychiatrists and team members for comprehensive care
- Maintaining required licenses, agreements, and insurance
- Upholding confidentiality, ethics, and professional standards
- Using evidence-based research to stay clinically competent
- Exhibiting strong communication, decision-making, and teamwork skills
- Performing comprehensive assessments, management, and referrals as per standards
Addiction-Specific Competencies:
- Understanding neurobiology of addiction
- Assessing and managing clients with addiction and co-morbid conditions
- Using advanced assessments and evidence-based interventions
- Providing recovery-oriented, trauma-informed care addressing social determinants
- Engaging clients in strengths-based care to promote resilience
- Educating clients and families about addiction management
- Collaborating across sectors and advocating for clients
Health Care Services for clients without a provider:
- Providing healthcare to clients 16+ meeting eligibility criteria
- Leading development of expanded medical services
- Assessing and managing addiction and co-morbid health issues
- Using advanced assessments to identify emotional, behavioral, and health states
- Implementing evidence-based interventions including medication management
- Providing recovery-oriented, trauma-informed care
- Facilitating collaborative practice and advocacy
- Identifying and acquiring new knowledge and skills
Outreach & Community Partnerships:
- Partnering with community agencies and programs
- Integrating counseling and support services
- Coordinating care with healthcare partners for community plans
- Developing programs aligned with best practices and evidence
- Promoting integration of addiction medicine and primary care
- Offering education and consultation
REQUIREMENTS:
- Police Vulnerable Sector Check (recent within 6 months)
- Registration with College of Nurses of Ontario as RN (Extended Class)
- Completion of Prescribing Narcotics and Controlled Substances course
- CPR certification
- Valid driver’s license, insurance, vehicle access
- Demonstrated regular attendance
PREFERRED QUALIFICATIONS:
- 2-3 years community healthcare experience
- Experience with marginalized populations, mental health, substance use, and trauma
- Knowledge of community resources and barriers
- Proficiency with various treatment strategies and computer skills
- Master’s Degree
- Suboxone online training
- Experience in addiction medicine and harm reduction
- Bilingual in both official languages of Canada
