Job description

POSITION SUMMARY:

Nurse Practitioners build and expand upon the competencies of RNs in the General Class. NPs have, and demonstrate in practice, the competencies to use their legislated authority to diagnose, order and interpret diagnostic tests, prescribe pharmaceuticals, and perform procedures.

NPs may practice in a wide range of settings, including hospital in-patients, outpatient clinics, and community-based clinics. The focus of practice is health promotion and the treatment of health conditions through supportive, preventative, therapeutic, palliative, and rehabilitative means to attain or maintain optimal functioning (Nursing Act 1990).

All NPs collaborate with physicians, inter-professional team members, patients, and families to provide evidence-informed care grounded in professional, ethical, and legal standards. The NP integrates advanced nursing practice knowledge, health management, health promotion, disease/injury prevention, and relevant biomedical and psychosocial theories to deliver comprehensive, patient/family-centered care. Practice involves complex integration of competencies, reflecting highly developed critical thinking, clinical experience, and advanced education.

QUALIFICATIONS:

  • Masters in Nursing/Nurse Practitioner (MN-NP) or MSc in Nursing with successful completion of a recognized post-Master’s Acute Care Nurse Practitioner Certificate Program.
  • OR completion of NP-PHC program (Baccalaureate level) prior to 2000 and a Masters in Nursing or relevant healthcare field.
  • Appointment or eligibility for appointment with a School of Nursing in an academic institution.
  • Mandatory Liability Insurance meeting CNO requirements (=$5 million coverage per setting).
  • Registration with the College of Nurses of Ontario in the RN (EC) class, with no restrictions.
  • Current BCLS/HCP certification.
  • Experience in an acute care, tertiary, or teaching setting preferred.
  • Completion of a CNO-approved prescribing course for controlled substances.
  • Knowledge and sensitivity in caring for seniors and their families.
  • Training in Gentle Persuasive Approach is preferred.
  • Clinical expertise in the specific clinical service area.
  • Leadership, decision-making, problem-solving skills.
  • Knowledge of legislative and ethical responsibilities.
  • Excellence in interpersonal and customer service skills.
  • Conflict management skills.
  • Data management skills for evaluation and change processes.

NEPHROLOGY SPECIFIC QUALIFICATIONS:

  • 1-3 years of NP experience required.
  • 3-5 years of Nephrology or Acute medicine experience preferred.
  • CNA Certification in Nephrology preferred.
  • Additional education in Advanced Wound Care Management preferred.
  • Expertise in wound care practices preferred.

RESPONSIBILITIES:

  • Practice in accordance with legislation, standards, guidelines, and policies.
  • Incorporate knowledge of diversity, cultural safety, and determinants of health.
  • Use knowledge of development, pathophysiology, epidemiology, and other sciences in assessments and management.
  • Provide relevant education and information to patients/clients.
  • Promote safe care by mitigating harm and addressing risks.
  • Document clinical data accurately and timely.
  • Formulate diagnoses, order and interpret investigations, and assume follow-up accountability.
  • Prescribe pharmacotherapy within scope, counsel patients on treatments.
  • Manage acute and chronic illnesses holistically and within scope.
  • Refer to other providers when outside scope or competence.
  • Engage in evidence-informed practice, research, and quality improvement.
  • Develop and evaluate processes to ensure coordinated care.
  • Implement research-based innovations to improve care.
  • Evaluate NP practice outcomes.
  • Lead educational initiatives, foster a positive learning environment, and demonstrate clinical leadership.
  • Support and mentor colleagues and students.
  • Travel and work at all SJHH Dialysis Units.
